Pimples on the scrotum and foreskin with itching in the groin area?

Hello, Doctor Hsu.
I have been experiencing severe itching in my groin and scrotum for the past month, and sometimes I scratch until it bleeds.
In the last two weeks, I have noticed red, pimple-like bumps on my scrotum and foreskin, measuring about 0.5 to 1 centimeter.
They are itchy as well, and when I try to squeeze them, nothing comes out; they just feel a bit hard.
I have developed four of these bumps over the past two weeks.
I usually wash with mild soap, but I still experience itching, and I sometimes scratch until I break the skin, which is quite distressing.
What are these bumps? Why are they itchy? Will they resolve on their own? Besides seeing a doctor, what other treatments can I try?

Dr. Xu Weikai reply Urology

Hello, it is recommended that you seek medical attention as soon as possible.
The degreasing ability of soap is too strong.
It is advised not to use any soap or body wash when cleaning the glans.
I hope this information is helpful to you.

More Info

It sounds like you're experiencing a challenging and uncomfortable situation with your groin and scrotal area. The symptoms you've described—itching, red bumps, and skin irritation—can be caused by a variety of conditions. Let's break down the potential causes, treatments, and management strategies for your symptoms.

Possible Causes
1. Fungal Infections: One common cause of itching in the groin area is a fungal infection, such as tinea cruris (commonly known as jock itch). This condition often presents with red, itchy patches and can sometimes lead to bumps or pustules.

2. Contact Dermatitis: This occurs when the skin reacts to irritants or allergens, such as soaps, detergents, or even certain fabrics. Symptoms include itching, redness, and sometimes bumps.

3. Folliculitis: This is an inflammation of the hair follicles, which can occur in the groin area. It may present as red, itchy bumps that can resemble acne.

4. Pearly Penile Papules: These are small, harmless bumps that can appear on the penis and are often mistaken for something concerning. They are normal anatomical variations and do not require treatment.

5. Sexually Transmitted Infections (STIs): Certain STIs can cause bumps and itching in the genital area. Conditions like herpes or genital warts may present with similar symptoms.

6. Scabies or Lice: These parasitic infestations can cause intense itching and may lead to secondary infections from scratching.

Treatment Options
1. Topical Antifungals: If a fungal infection is suspected, over-the-counter antifungal creams (like clotrimazole or miconazole) can be effective. Apply as directed until symptoms resolve.

2. Hydrocortisone Cream: For itching and inflammation, a mild hydrocortisone cream can help reduce symptoms. However, it should not be used for extended periods without medical supervision.

3. Antihistamines: Oral antihistamines may help alleviate itching, especially if the cause is allergic in nature.

4. Proper Hygiene: Continue to maintain good hygiene by washing the area with mild soap and ensuring it is dry. Avoid tight clothing that may irritate the skin.

5. Avoid Scratching: As difficult as it may be, try to avoid scratching the area to prevent further irritation and potential infection.

When to See a Doctor
While some conditions may resolve on their own, it is essential to consult a healthcare professional if:
- The bumps persist or worsen.

- You experience significant pain or discomfort.

- There are signs of infection (increased redness, warmth, swelling, or discharge).

- You develop fever or systemic symptoms.

- You suspect an STI.

Conclusion
In summary, the symptoms you're experiencing could be due to a variety of causes, including fungal infections, contact dermatitis, or other dermatological conditions. While some treatments can be initiated at home, it is crucial to seek medical advice for a proper diagnosis and tailored treatment plan. A healthcare provider can perform a physical examination and may recommend tests to determine the exact cause of your symptoms. Remember, early intervention can lead to better outcomes and relief from discomfort.
