Anal pain?

I have been experiencing issues with irregular bowel movements.
Although I do have a bowel movement every day upon waking up, due to poor habits developed since childhood, it takes me at least 40 minutes each time.
Recently, I have noticed that after using the restroom, there is a small amount of bleeding when I wipe, and I also feel pain around the anus.
What could be the cause of this?

Dr. Chen Qifeng reply Gastroenterology and Hepatology

Hello: Patients with constipation often experience complications such as anal pain and bleeding.
It is recommended to visit a colorectal surgery specialist for further evaluation and treatment.
The most common cause is hemorrhoids, but other conditions may also be present.
Wishing you safety and good health.

Anal pain and bleeding can be concerning symptoms, and they often indicate underlying issues that require attention. Based on your description, it seems that you have been experiencing constipation and prolonged straining during bowel movements, which can lead to several complications, including hemorrhoids, anal fissures, or other conditions affecting the anal region.

Causes of Anal Pain and Bleeding:
1. Hemorrhoids: These are swollen veins in the lower rectum or anus that can cause pain, itching, and bleeding, especially during bowel movements. Straining due to constipation can exacerbate this condition.

2. Anal Fissures: These are small tears in the lining of the anus, often resulting from passing hard stools. They can cause sharp pain during and after bowel movements and may lead to bleeding.

3. Constipation: Chronic constipation can lead to straining, which increases the risk of developing hemorrhoids and fissures. It can also cause discomfort and pain in the anal area.

4. Infections or Inflammatory Conditions: Conditions such as anal abscesses or inflammatory bowel disease (IBD) can also lead to pain and bleeding.

5. Other Conditions: Less commonly, anal pain and bleeding can be associated with more serious conditions, such as rectal polyps or colorectal cancer, especially if there are changes in bowel habits or if the bleeding is significant.

Solutions and Recommendations:
1. Dietary Changes: Increasing your fiber intake can help soften stools and promote regular bowel movements. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Additionally, staying well-hydrated is crucial.

2. Regular Bowel Habits: Try to establish a routine for bowel movements. Avoid straining and give yourself enough time to use the bathroom without rushing.

3. Over-the-Counter Treatments: For hemorrhoids, topical treatments such as creams or ointments can provide relief from pain and itching. Stool softeners or mild laxatives can help alleviate constipation.

4. Warm Sitz Baths: Soaking in warm water can help relieve pain and discomfort in the anal area. This can be particularly soothing if you have hemorrhoids or fissures.

5. Avoiding Irritants: Be cautious with the use of toilet paper; consider using moist wipes or a bidet to reduce irritation.

6. Medical Consultation: Given that you are experiencing bleeding and pain, it is essential to consult a healthcare professional. They can perform a physical examination and may recommend further tests, such as a colonoscopy, to rule out any serious conditions.

7. Follow-Up: If your symptoms persist or worsen, follow up with your healthcare provider. They may refer you to a specialist, such as a gastroenterologist or a colorectal surgeon, for further evaluation and treatment.

In conclusion, while anal pain and bleeding can often be attributed to benign conditions like hemorrhoids or anal fissures, it is crucial to seek medical advice to rule out more serious issues. Making dietary and lifestyle changes can significantly improve your symptoms, but professional guidance is essential for proper diagnosis and treatment.
