Meniere's disease

Hello Doctor, I would like to ask again.
I originally had sleep issues, but the examination results showed that my respiratory control is good.
However, the frequency of waking up at night and the feeling of fatigue upon waking have not improved at all.
Shortly after getting up, I feel drowsy again.
I have a sensation of imbalance in the Eustachian tube (when I hear loud sounds, my ears ring, and I occasionally experience tinnitus).
I feel sleepy and slightly dizzy (but I can move normally, though my cognitive function seems impaired), and I have difficulty concentrating.
Recently, I discovered that my symptoms seem to align with those of sphenoid/ethmoid sinusitis, as I experience discomfort and a tight sensation in the upper eye socket, nasal bridge, and between my eyebrows, with tenderness in that area.
There is slight dull pain in my head or back of the head.
However, I do not have much nasal discharge, but I experience post-nasal drip and occasionally cough at night with some yellow-green phlegm.
I have had nasal congestion consistently from bedtime to waking up.
However, after visual and X-ray examinations, the doctor believes it is not a sinusitis issue.
The prescribed medications include three types for Meniere's disease and those for controlling allergies, as well as antibiotics.
Could you please advise if the fatigue, dull headache, drowsiness, and lack of concentration are related to Meniere's disease? Besides chronic sinusitis, what other conditions could present with similar symptoms? Thank you.

XOF, 40~49 year old female. Ask Date: 2015/04/16

Dr. Zheng Jueyi reply Otolaryngology

Hello: If you have already been examined and it is likely not related to sinusitis, symptoms such as fatigue and drowsiness may be more associated with sleep issues.
It is recommended to consult a psychiatrist.
Additionally, some medications for Meniere's disease and those for controlling allergies can also cause drowsiness or difficulty concentrating.
It is advisable to discuss this further with your physician.
Wishing you safety and good health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

Meniere's disease is a chronic inner ear disorder that can significantly affect an individual's quality of life. It is characterized by episodes of vertigo, tinnitus (ringing in the ears), and fluctuating hearing loss. The exact cause of Meniere's disease is not fully understood, but it is believed to be related to an abnormal accumulation of fluid in the inner ear, which can disrupt the normal functioning of the vestibular and auditory systems.

Symptoms of Meniere's Disease
The primary symptoms of Meniere's disease include:
1. Vertigo: This is often the most debilitating symptom, characterized by a sensation of spinning or dizziness that can last from minutes to hours. During an episode, individuals may feel a loss of balance, which can lead to falls or injuries.

2. Tinnitus: Many patients report a persistent ringing, buzzing, or roaring sound in the affected ear. This can be constant or intermittent and may vary in intensity.

3. Hearing Loss: The hearing loss associated with Meniere's disease often fluctuates, meaning it can improve and worsen over time. In the early stages, it may be temporary, but as the disease progresses, it can lead to permanent hearing loss.

4. Aural Fullness: Patients often describe a feeling of pressure or fullness in the affected ear, which can be uncomfortable.

Impact on Daily Life
The symptoms of Meniere's disease can severely impact daily activities and overall quality of life. The unpredictability of vertigo attacks can lead to anxiety and fear of engaging in social situations or activities that may trigger an episode. Individuals may find it challenging to maintain employment, especially in jobs that require physical activity or concentration. The combination of fatigue, cognitive impairment, and emotional distress can further complicate daily functioning.

Relationship with Other Conditions
In your case, you mentioned experiencing fatigue, sleep disturbances, and cognitive difficulties, which could be related to several factors. While Meniere's disease can contribute to these symptoms, it is essential to consider other potential underlying conditions. For example:
- Chronic Sinusitis: Symptoms like facial pain, pressure, and nasal congestion can lead to sleep disturbances and fatigue. If your doctor has ruled out sinusitis, it may be worth exploring other forms of upper respiratory issues.

- Sleep Disorders: Conditions such as sleep apnea can lead to excessive daytime sleepiness and cognitive difficulties. Even if your breathing was reported as normal, other sleep-related issues could still be present.

- Allergies: Allergic reactions can cause nasal congestion and fatigue, which may exacerbate feelings of tiredness and cognitive impairment.

- Medication Side Effects: Some medications used to treat Meniere's disease, such as diuretics or antihistamines, can cause drowsiness and cognitive issues as side effects.

Conclusion
It is crucial to have a comprehensive evaluation by a healthcare professional to determine the exact cause of your symptoms. A multidisciplinary approach may be beneficial, involving an ENT specialist for Meniere's disease, a sleep specialist for potential sleep disorders, and possibly an allergist for allergy-related issues. Keeping a symptom diary can also help identify triggers and patterns, which can be valuable information for your healthcare team.

In summary, while Meniere's disease can contribute to symptoms like fatigue, cognitive difficulties, and tinnitus, it is essential to explore other potential causes to address your concerns effectively.
