Urethral itching

Recently, I have been experiencing pain in the lower abdomen, but pressing on it does not cause discomfort.
Occasionally, I feel a burning sensation when urinating, but this only occurs during the first morning urine or when the urine volume is low.
At other times, everything seems normal.
The urination process is very normal, and the urine volume is also normal, although I do experience occasional frequency.
The day before yesterday, I had a sensation of itching in the urethra and at the urethral opening.
During the process, if I change positions from sitting to lying down, a few drops of urine may leak out.
A few days ago, I also felt swelling in the perineum and behind the testicles.
I am unsure if the recent lower back pain and leg soreness are related.
I hope the doctor can help clarify this.
Thank you.

Urethral itching and associated symptoms can be indicative of various underlying conditions, particularly in the context of urology. Your description of symptoms, including lower abdominal discomfort, occasional burning during urination, urethral itching, and the sensation of swelling in the perineum and behind the testicles, raises several considerations.

Firstly, the burning sensation during urination, especially if it occurs primarily with the first morning urine or when the urine volume is low, may suggest a urinary tract infection (UTI) or irritation of the urethra. UTIs are common and can cause symptoms such as burning, urgency, frequency, and sometimes even lower abdominal pain. The fact that you experience these symptoms primarily in the morning could be due to the concentration of urine after a night of not urinating, which can irritate the bladder and urethra.

The itching sensation in the urethra and at the urethral orifice can also be associated with infections, such as a UTI or sexually transmitted infections (STIs), including chlamydia or gonorrhea. Itching can also result from irritation due to soaps, lotions, or other products that come into contact with the genital area. If there is any discharge accompanying the itching, it would be important to seek medical attention for further evaluation.

The sensation of swelling in the perineum and behind the testicles could indicate several things, including inflammation or infection in the area, such as epididymitis (inflammation of the epididymis) or prostatitis (inflammation of the prostate gland). Both conditions can cause discomfort and may be associated with urinary symptoms. Epididymitis, in particular, can present with swelling, pain, and sometimes fever, and it is often linked to infections.

As for the lower back pain and leg discomfort, these symptoms can sometimes be related to urological issues, particularly if there is any involvement of the kidneys or if there is referred pain from the pelvic region. Conditions such as kidney stones or infections can cause pain that radiates to the back and legs.

Given the complexity of your symptoms, it is crucial to consult a healthcare professional for a thorough evaluation. A urologist would likely perform a physical examination, obtain a detailed medical history, and may recommend tests such as urinalysis, urine culture, or imaging studies to determine the underlying cause of your symptoms.
In summary, your symptoms could be related to a urinary tract infection, irritation, or inflammation of the urethra, or other urological conditions. It is essential to seek medical attention to receive an accurate diagnosis and appropriate treatment. Early intervention can help prevent complications and alleviate discomfort.
