Sudden onset of testicular pain?
Hello doctor, a few days ago before going to sleep, I turned over and suddenly felt pain in my left testicle.
The pain has persisted for 2 to 3 days, sometimes it’s absent, but at times there is a slight pain (walking or jostling also causes a bit of discomfort).
On the third day, I examined it closely and found a small round lump above the testicle, which is painful to touch.
The pain feels like a dull ache, similar to being hit by a ball.
This area has been hurting for the past few days, but I didn’t notice the lump before.
Could this be testicular torsion or something else? (I masturbate almost every two days.)

Sudden testicular pain can be concerning and may arise from various causes, some of which require immediate medical attention. Based on your description, it seems you are experiencing persistent pain in your left testicle, accompanied by a small painful lump above it. This situation warrants careful consideration of potential causes.
One of the most serious conditions that can cause sudden testicular pain is testicular torsion. This occurs when the spermatic cord becomes twisted, cutting off the blood supply to the testicle. Symptoms typically include sudden, severe pain, swelling, and sometimes nausea or vomiting. Testicular torsion is a medical emergency, and if not treated promptly, it can lead to permanent damage or loss of the testicle. However, you mentioned that the pain has been intermittent and not as severe as one might expect with torsion, which is somewhat reassuring but does not eliminate the need for evaluation.
Another possible cause of your symptoms could be epididymitis, which is an inflammation of the epididymis, often due to infection. This condition can cause pain and swelling in the testicle and may be accompanied by fever or discharge. Given your history of frequent masturbation, it's worth noting that sexually transmitted infections (STIs) can also lead to epididymitis, especially in younger men.
A less common but still relevant possibility is a hernia, where tissue protrudes through a weak spot in the abdominal muscles, potentially causing discomfort in the groin or testicular area. Additionally, trauma to the area, even if minor, can lead to bruising or swelling that might explain the lump and pain you are experiencing.
Given the presence of a lump and persistent pain, it is crucial to seek medical evaluation as soon as possible. A healthcare provider will likely perform a physical examination and may order an ultrasound to assess blood flow to the testicle and to evaluate the lump. This imaging can help differentiate between torsion, epididymitis, or other potential issues.
In the meantime, avoid any activities that could exacerbate the pain, such as heavy lifting or vigorous exercise. Over-the-counter pain relief, such as ibuprofen or acetaminophen, may help manage discomfort, but it is essential to address the underlying cause.
In summary, while there are several potential causes for your symptoms, including testicular torsion, epididymitis, or a hernia, the presence of a painful lump and persistent discomfort necessitates prompt medical evaluation. Early diagnosis and treatment are critical to prevent complications and ensure the best possible outcome for your health. Please do not delay in seeking care.
