Prostate. Slow urination and related sexual dysfunction issues?
Hello Dr.
Hsu,
In recent years, I have been experiencing very slow urination.
Sometimes it takes a long time to urinate, even waiting 3 to 5 minutes before I can go.
This has been quite bothersome.
About 5 to 6 years ago, I consulted a doctor who diagnosed me with benign prostatic hyperplasia (BPH).
However, the medication I was prescribed did not bring any improvement or relief, so I did not follow up with further consultations.
During this period, I did not experience any significant difficulties with urination, although the frequency varied—sometimes it was more frequent, and other times it seemed normal.
Overall, the slow urination is what troubles me the most.
Despite seeing doctors and taking medication, I have not noticed any significant effects, which has left me feeling quite discouraged.
This issue seems to greatly affect male sexual function, as I often experience insufficient blood flow and inadequate firmness, which is very distressing.
I would appreciate your guidance on this matter.
After many years of outpatient treatment with little improvement or relief, it feels like I can only watch the situation worsen, much like being trapped in a city of despair.
I look forward to your response and advice.
Thank you in advance!
Sincerely,

Hello, first, a detailed assessment can be conducted to determine if the symptoms are indeed caused by benign prostatic hyperplasia (BPH).
If the prostate is significantly enlarged, a definitive surgical option such as prostatectomy may be considered.
If there are concerns about surgical bleeding, laser treatment could be an alternative.
If the prostate is of normal size, medication therapy is still necessary.
In recent years, various medications for BPH have been developed, and you might want to try them.
Additionally, erectile dysfunction should also be evaluated to check for potential issues related to testosterone deficiency or neurological and vascular problems.
Note: Both of your symptoms can be somewhat improved through regular exercise, which helps stabilize the autonomic nervous system.
If you have further questions, feel free to ask for more discussion and research.
Online responses should only serve as a reference; it is essential to seek medical attention at a healthcare facility if necessary.

Understanding slow urination, especially in men, can be a complex issue often linked to various underlying health conditions. In your case, it seems that you have been experiencing slow urination for several years, which has led to significant discomfort and concern regarding its impact on your overall health and well-being.
The symptoms you describe, such as taking several minutes to urinate and experiencing a feeling of incomplete bladder emptying, are commonly associated with benign prostatic hyperplasia (BPH), or prostate enlargement. This condition is prevalent among older men and can lead to urinary symptoms like difficulty starting urination, a weak urine stream, and prolonged urination time. While you mentioned that you had previously consulted a doctor and were prescribed medication without significant improvement, it is essential to understand that not all treatments work for every individual, and sometimes a different approach may be necessary.
In addition to BPH, other potential causes of slow urination can include urinary tract infections (UTIs), bladder stones, or even neurological conditions that affect bladder function. Given that you have not seen a doctor for this issue in some time, it may be beneficial to schedule a follow-up appointment with a urologist. They can perform a thorough evaluation, which may include a physical examination, urine tests, and possibly imaging studies to assess the prostate and bladder.
Regarding your concerns about how slow urination may affect sexual function, it is not uncommon for men with urinary issues to experience related sexual health problems, such as erectile dysfunction or reduced libido. This can be due to a combination of physical factors, such as hormonal changes or blood flow issues, and psychological factors, including anxiety or stress related to urinary difficulties. Addressing the underlying urinary issue may help alleviate some of these sexual health concerns.
If you have been feeling discouraged due to the lack of improvement, it is important to remember that there are various treatment options available for BPH and other urinary issues. These can range from lifestyle modifications, such as dietary changes and pelvic floor exercises, to medications that can help relax the bladder neck and improve urine flow. In more severe cases, surgical options may be considered to relieve symptoms.
In summary, while slow urination can be a frustrating and concerning issue, it is essential to seek further medical evaluation to determine the underlying cause and appropriate treatment. Do not hesitate to reach out to a healthcare professional who specializes in urology, as they can provide you with the guidance and support you need to manage your symptoms effectively. Remember, you are not alone in this, and there are solutions available that can improve your quality of life.
