Itchy nipples?

Recently, I've started experiencing sudden itching in my nipples.
It's unbearable, and I find myself scratching, which is very uncomfortable.
There is also some discharge.
What could this be? Thank you.

Xiao Xiao, 10~19 year old female. Ask Date: 2015/06/06

Dr. Zhou Jiazhen reply Breast and Thyroid

Hello, Xiao Xiao:
The issue is mostly caused by old secretions accumulating on the surface of the nipple.
Sometimes, there may be invisible tiny secretions on the nipple that can cause itching and discomfort.
In such cases, a topical ointment containing a small amount of steroids may be applied.
If the symptoms persist, it is recommended to follow up at an outpatient clinic.

Wishing you safety and health,
Director of Surgery, Taoyuan General Hospital, Ministry of Health and Welfare

More Info

Itchy nipples can be a source of discomfort and concern, and understanding the potential causes is essential for addressing the issue effectively. The sensation of itchiness in the nipple area can arise from various factors, ranging from benign skin conditions to more serious underlying health issues. Here’s a comprehensive overview of the possible causes and when to seek medical help.

Common Causes of Itchy Nipples
1. Skin Irritation: One of the most common reasons for itchy nipples is skin irritation. This can occur due to friction from clothing, particularly if you wear bras that are too tight or made from irritating materials. Additionally, changes in weather, such as dry air in winter, can lead to dry skin, resulting in itchiness.

2. Allergic Reactions: Allergies to soaps, detergents, lotions, or even fabrics can cause localized itching. If you have recently changed any products that come into contact with your skin, this could be a potential cause.

3. Eczema or Dermatitis: Conditions like eczema or contact dermatitis can lead to itchy, inflamed skin. These conditions may also cause redness, scaling, or flaking of the skin around the nipples.

4. Hormonal Changes: Hormonal fluctuations, particularly those related to the menstrual cycle, pregnancy, or breastfeeding, can lead to changes in the skin and sensitivity, resulting in itchiness.

5. Infections: Fungal infections, such as candidiasis, can occur in warm, moist areas of the body, including the nipple region. Bacterial infections can also cause irritation and discharge.

6. Paget's Disease of the Nipple: This is a rare form of breast cancer that begins at the nipple and can cause symptoms such as itching, redness, and discharge. It is essential to be aware of this condition, especially if the itchiness is accompanied by other unusual symptoms.

7. Nipple Discharge: If you are experiencing discharge along with itchiness, it could indicate an underlying issue that requires medical evaluation. Discharge can be a sign of infection or other breast conditions.

When to Seek Medical Help
While occasional itchiness may not be a cause for alarm, certain signs and symptoms warrant a visit to a healthcare provider:
- Persistent Itchiness: If the itchiness does not resolve with home care measures or persists for more than a few days.

- Accompanying Symptoms: If you notice redness, swelling, or any unusual discharge from the nipple, it is essential to seek medical advice.

- Changes in Appearance: Any changes in the appearance of the nipple or surrounding skin, such as scaling, crusting, or the development of lumps, should be evaluated by a healthcare professional.

- Pain or Discomfort: If the itchiness is accompanied by pain or discomfort, it is advisable to consult a doctor.

Conclusion
Itchy nipples can arise from various causes, many of which are benign and easily treatable. However, it is crucial to pay attention to accompanying symptoms and seek medical advice when necessary. A healthcare provider can perform a thorough evaluation, including a physical examination and possibly additional tests, to determine the underlying cause of the itchiness and recommend appropriate treatment. In the meantime, practicing good hygiene, wearing breathable fabrics, and avoiding known irritants can help alleviate discomfort.
