Experiencing blood on toilet paper after a bowel movement occasionally over a long period?

Hello Dr.
Chen, in March 2014, I took antibiotics for vaginitis and experienced bloody stools, after which I stopped the medication.
Subsequently, I developed anal fissures and noticed blood when wiping, along with occasional discharge of orange, foul-smelling fluid.
The condition improved, and I didn't pay much attention to it while traveling abroad for an extended period.
To this day, I occasionally experience anal fissures (a tearing sensation) when passing somewhat hard stools, or a sharp, rubbing pain inside the anal area, along with blood when wiping.
Could this be hemorrhoids? Should I seek medical attention? (I don't want to waste healthcare resources) Thank you.

Miss Chen, 20~29 year old female. Ask Date: 2015/06/12

More Info

When it comes to the presence of blood after wiping, especially in the context of a history of anal fissures and potential hemorrhoids, it is essential to approach the situation with caution and a thorough understanding of the possible underlying conditions.
Based on your description, it seems that you have experienced a series of gastrointestinal issues, including a history of antibiotic use leading to vaginal inflammation, which may have contributed to your current symptoms. The presence of blood after wiping, particularly when accompanied by a sensation of tearing or pain during bowel movements, raises the possibility of anal fissures or hemorrhoids.
Anal fissures are small tears in the lining of the anus, often caused by passing hard stools, which can lead to sharp pain and bleeding during bowel movements. The blood you observe after wiping could be bright red, indicating that it is fresh and likely coming from the anal area rather than from higher up in the gastrointestinal tract. This is typically a sign of localized issues such as fissures or hemorrhoids.
On the other hand, hemorrhoids are swollen veins in the lower rectum or anus that can cause discomfort, itching, and bleeding. They can be internal or external, and while they may not always cause pain, they can lead to bleeding during bowel movements, especially if there is straining involved. The orange-colored liquid you mentioned could be a sign of mucus discharge, which can occur with hemorrhoids or anal fissures.

Given your symptoms, it is advisable to seek medical attention. While you may be concerned about utilizing healthcare resources, it is crucial to prioritize your health. A healthcare professional can conduct a thorough examination, which may include a visual inspection and possibly a digital rectal exam, to determine the exact cause of your symptoms. They may also recommend treatments such as dietary changes to include more fiber, stool softeners, topical treatments for pain relief, or other interventions depending on the diagnosis.

In the meantime, consider implementing some self-care measures. Increasing your fiber intake through fruits, vegetables, and whole grains can help soften your stools and reduce the strain during bowel movements. Staying well-hydrated is also essential. Over-the-counter topical treatments may provide relief from discomfort associated with hemorrhoids or fissures.
In conclusion, while your symptoms may suggest hemorrhoids or anal fissures, a definitive diagnosis can only be made through a medical evaluation. It is important to address these symptoms promptly to prevent further complications and to ensure that you receive appropriate treatment. Your health is worth the investment, and seeking medical advice is a proactive step towards recovery.
