Post-banding of internal hemorrhoids

Hello, doctor.
I have two questions to ask:
1.
I previously underwent rubber band ligation for internal hemorrhoids, and today the rubber band fell off, but I have started to notice yellow discharge.
Is this normal?
2.
I have been taking a stool softener, but for the past few days, my bowel movements have not been smooth.
I often feel like I am at the opening but cannot pass it, and I cannot strain to push it down.
It has been two days since my last bowel movement.
What should I do in this situation? Is this normal?

Niming, 20~29 year old female. Ask Date: 2015/06/17

Dr. Ke Fangxu reply Surgery

It is normal as long as there is no severe pain or significant bleeding.
Generally, the discharge will subside within about a week.
After hemorrhoid ligation, there may be some abnormal sensations due to irritation from the wound.
It is recommended to take warm sitz baths, which should help alleviate this condition.
If constipation persists, it is advisable to increase the dosage of stool softeners.

Post-banding care for internal hemorrhoids is an important aspect of recovery, and it’s understandable to have concerns following the procedure. Let's address your questions one by one.

1. Yellow Discharge After Rubber Band Ligation: It is not uncommon to experience some discharge after the rubber band ligation of internal hemorrhoids. The yellow discharge you are noticing could be a result of the healing process. After the band is placed, the tissue around the hemorrhoid may become inflamed and produce discharge as it heals. However, if the discharge is accompanied by a foul odor, increased pain, or fever, it could indicate an infection, and you should contact your healthcare provider immediately.

2. Difficulty with Bowel Movements: It is also common to experience changes in bowel habits after the procedure. The sensation of having a bowel movement but being unable to pass stool can be frustrating and uncomfortable. This may be due to several factors, including the swelling of the tissue around the treated area, the effects of the rubber band, or even anxiety about pain during bowel movements. Here are some recommendations to help manage this situation:
- Dietary Adjustments: Ensure you are consuming a high-fiber diet, which includes plenty of fruits, vegetables, and whole grains. This can help soften the stool and make it easier to pass.

- Hydration: Drink plenty of fluids, as staying well-hydrated can also aid in preventing constipation.

- Stool Softeners: Continue using stool softeners as prescribed. If you find that they are not effective, consult your doctor about adjusting the dosage or trying a different product.

- Gentle Movement: Engaging in light physical activity, such as walking, can stimulate bowel function and help ease the passage of stool.

- Avoid Straining: It’s crucial to avoid straining during bowel movements, as this can exacerbate hemorrhoid symptoms and lead to complications. If you feel the urge to go but cannot, try to relax and give it some time before attempting again.

If you have not had a bowel movement for two days and are experiencing significant discomfort, it may be advisable to reach out to your healthcare provider. They may recommend additional interventions, such as a mild laxative or an enema, to help relieve constipation.

In summary, while some discharge and changes in bowel habits can be normal after rubber band ligation for internal hemorrhoids, it’s essential to monitor your symptoms closely. If you notice any signs of infection or if your bowel habits do not improve, do not hesitate to seek medical advice. Your healthcare provider can offer tailored recommendations based on your specific situation and ensure that your recovery progresses smoothly.
