Can rabies be transmitted indirectly?

Hello, Dr.
Chen.
Today, I went to a café in Taipei with my girlfriend, where there were many cats.
My girlfriend loves to play with the cats, and she touched a cat and then touched my head.
I often have scalp lesions that resemble pimples (I'm not sure if she touched them).
I'm concerned whether her hands might have come into contact with cat saliva, which could lead to indirect infection.
Additionally, I have other wounds on my body, and while I don't feel like I was licked by a cat, I'm unsure if I might have indirectly come into contact with areas that were licked or if I was inadvertently licked.
Should I still be concerned about rabies? Is Taipei or Taiwan considered an endemic area? I'm uncertain whether I need to get vaccinated or visit a hospital for examination.
I would like to consult with you, doctor.
Thank you very much!

Hello,
Thank you for your inquiry regarding your concerns about the potential transmission of rabies from direct or indirect contact with unknown cats.
Currently, there is no rabies outbreak among stray cats and dogs in our country, and there are also no reported cases of rabies in wildlife in the northern region.
Therefore, you do not need to be overly concerned about your situation.
When playing with cats, please take care to avoid having any wounds licked.
If you do get licked or scratched, simply clean the area with standard disinfecting procedures and monitor it.
I hope this information is helpful to you.

Rabies is a viral disease that primarily affects mammals, including cats, and is transmitted through the saliva of an infected animal, typically via bites. The concern you have regarding potential rabies transmission through indirect contact with cats is valid, especially if there are open wounds involved. However, let's clarify a few key points regarding rabies transmission and the specific situation you described.

1. Transmission of Rabies: Rabies is primarily transmitted through bites or scratches from an infected animal. The virus is present in the saliva of an infected animal, and it is most commonly transmitted when saliva enters the body through broken skin or mucous membranes. Indirect transmission, such as through contaminated surfaces or hands, is not considered a common route for rabies transmission.
2. Your Situation: In your case, if your girlfriend touched a cat and then touched your head, the risk of rabies transmission would be extremely low unless the cat was rabid and had saliva on her hands that came into contact with your open wounds. If there was no direct bite or scratch from the cat, and if you did not see any signs of rabies in the cat (such as unusual behavior, excessive drooling, or paralysis), the risk remains minimal.

3. Rabies in Taiwan: As of my last knowledge update in October 2023, Taiwan has made significant progress in rabies control, particularly in domestic animals. However, rabies is still a concern in certain areas, especially with wild animals. It's important to be aware of the local epidemiology of rabies, but urban areas like Taipei generally have lower risks compared to rural areas.

4. When to Seek Medical Attention: If you have open wounds and are concerned about potential exposure to rabies, it is advisable to consult a healthcare professional. They may recommend post-exposure prophylaxis (PEP) if there is a significant risk of rabies exposure. This typically involves a series of rabies vaccinations and, in some cases, rabies immune globulin.

5. Preventive Measures: To minimize the risk of rabies and other infections, it is always a good practice to wash hands thoroughly after interacting with animals, especially if you have open wounds. Keeping wounds clean and covered can also help prevent infections.

In summary, while the risk of rabies transmission through indirect contact is low, it is always better to err on the side of caution, especially if you have open wounds. If you have any doubts or if your girlfriend had direct contact with a cat that is known to be rabid, seeking medical advice is the best course of action. Always stay informed about local health advisories regarding rabies and other zoonotic diseases.

Hello, rabies is primarily transmitted through animal bites. In rare cases, infection can occur without an animal bite, such as through inhalation of viral particles. The infection rate in the situation you mentioned should be extremely low.

Animal saliva becomes infectious only 10 days before the onset of illness. If the animal does not show symptoms, the infectious potential is very low. Additionally, whether an infection occurs depends on whether there are any wounds or cavities in the child's mouth. Overall,...
