Treatment of sebaceous cysts?
I am experiencing issues with multiple sebaceous glands.
Are there any effective treatments available, such as surgery or carbon dioxide laser therapy that I found online, or are there other methods? Oral isotretinoin has not worked for me.
I would appreciate your guidance, doctor.

There are no effective treatment options; carbon dioxide laser can be used for treatment, but it has a high recurrence rate, and lesions may appear in other areas as well.
Generally, aggressive treatment is not recommended.

Sebaceous cysts, also known as epidermoid cysts, are common benign growths that can occur anywhere on the body, including the face, neck, and trunk. They are typically filled with a thick, yellowish substance called keratin, which is produced by the skin cells. While these cysts are usually harmless, they can become bothersome if they become inflamed, infected, or if they grow large enough to cause discomfort or cosmetic concerns.
When it comes to effective treatments for sebaceous cysts, there are several options available, each with its own advantages and disadvantages. Here’s a detailed overview of the most common treatment methods:
1. Surgical Excision: This is the most definitive treatment for sebaceous cysts. During this procedure, a surgeon will make an incision over the cyst, remove it completely along with its capsule, and then close the incision with stitches. This method is effective because it removes the cyst entirely, reducing the likelihood of recurrence. However, it does involve a recovery period and potential scarring.
2. Incision and Drainage: If a cyst becomes infected or inflamed, a doctor may perform an incision and drainage procedure. This involves making a small cut in the cyst to allow the contents to drain out. While this can provide immediate relief from pain and pressure, it does not remove the cyst itself, and there is a high chance of recurrence.
3. Laser Therapy: Carbon dioxide (CO2) laser treatment is another option that can be used to remove sebaceous cysts. This method uses focused light energy to vaporize the cyst. Laser therapy can be less invasive and may result in less scarring compared to traditional surgical excision. However, it may not be suitable for all cysts, especially larger ones, and the effectiveness can vary.
4. Injection of Steroids: In some cases, a doctor may inject a steroid medication into the cyst to reduce inflammation and swelling. This can be effective for cysts that are inflamed but does not remove the cyst itself.
5. Topical Treatments: While topical treatments like retinoids (e.g., topical tretinoin) may help prevent new cysts from forming, they are generally not effective for treating existing cysts. Oral isotretinoin (Acutane) is another option, but it is typically reserved for severe cases of acne and may not be effective for sebaceous cysts.
6. Observation: If the cyst is not causing any symptoms or cosmetic concerns, a "watch and wait" approach may be appropriate. Many sebaceous cysts do not require treatment unless they become problematic.
In terms of effectiveness, surgical excision is often considered the gold standard for treating sebaceous cysts, as it provides a permanent solution. Laser therapy can be effective for smaller cysts and may be preferred for patients concerned about scarring. Incision and drainage can provide quick relief but may lead to recurrence.
It's essential to consult with a healthcare professional to determine the best treatment option based on the cyst's size, location, and symptoms. They can provide personalized recommendations and discuss potential risks and benefits associated with each treatment method.
In conclusion, if you are experiencing multiple sebaceous cysts, surgical excision is likely the most effective treatment to consider, especially if you are looking for a long-term solution. Laser therapy may also be a viable option, particularly for those concerned about scarring. Always consult with a dermatologist or a qualified healthcare provider to explore the best treatment tailored to your specific condition.
