After taking traditional Chinese medicine, the urine may appear yellow?

I experienced lower abdominal pain due to constipation and consulted a Traditional Chinese Medicine (TCM) physician who prescribed a formula consisting of Xiao Jian Zhong Tang (8.0g), Tiao Wei Cheng Qi San (5.0g), and Geng Mi (0.5g).
After taking the medication yesterday afternoon, I had a bowel movement this morning, but my urine has been very yellow despite drinking plenty of water.
Typically, my urine only appears this yellow after just waking up or after taking B vitamins, but I haven't taken any B vitamins recently.
Is this a normal reaction to these medications? Thank you!

Dr. Yan Sumei reply Traditional Chinese Medicine

Dear online friend, Ma Mian: Your prescription includes Da Huang in the Chao Wei Cheng Qi San formula.
This herbal ingredient can cause your urine to turn a deep yellow color.
Once you stop taking the medication, the urine color will return to normal.
This is your response.
Wishing you health and safety! Sincerely, Yen Su-Mei, Department of Traditional Chinese Medicine, Nantou Hospital.

More Info

Yellow urine after taking Traditional Chinese Medicine (TCM) can indeed be a normal phenomenon, particularly when certain herbal ingredients are involved. In your case, the prescription you received includes Xiao Jian Zhong Tang and Tiao Wei Cheng Qi San, which are known to contain ingredients that can affect urine color.
One of the key components in Tiao Wei Cheng Qi San is Da Huang (Rhubarb), which is known for its purgative properties. Da Huang can cause changes in urine color, leading to a darker or more yellow appearance. This is primarily due to the presence of anthraquinones, which are natural compounds found in rhubarb that can impart a yellow or reddish hue to urine. This effect is generally harmless and should resolve once the medication is discontinued.

Additionally, the color of urine can be influenced by hydration levels. You mentioned that you have been drinking a lot of water, which typically dilutes urine and can lead to a lighter color. However, if the herbal components are potent enough, they can still cause the urine to appear yellow despite high fluid intake.
It's also worth noting that certain medications and supplements can lead to changes in urine color. For instance, B vitamins, particularly B2 (riboflavin), are known to cause bright yellow urine. Since you indicated that you have not been taking B vitamins recently, it is less likely that this is the cause of your yellow urine.

In summary, the yellow urine you are experiencing after taking the prescribed TCM is likely a normal reaction to the herbal ingredients, particularly Da Huang. As long as you are not experiencing any other concerning symptoms, such as pain, burning during urination, or any signs of infection, this change in urine color should not be a cause for alarm.
If the yellow color persists after stopping the medication or if you develop any new symptoms, it would be prudent to consult with your healthcare provider for further evaluation. They can help determine if there are any underlying issues that need to be addressed. Always ensure that you communicate any changes in your health to your healthcare provider, especially when undergoing treatment with herbal medicines, as they can interact with other medications and affect your overall health.
In conclusion, while yellow urine can be alarming, in the context of TCM and the specific herbs you are taking, it is likely a benign side effect. However, maintaining open communication with your healthcare provider is essential for ensuring your health and well-being.
