Bloating and left lower abdominal pain?

Hello Dr.
Chen, I have had gastrointestinal issues since childhood, including constipation and bloating.
Recently, whenever I feel hungry, I experience pain in my stomach (in the upper middle area), but once I eat, my stomach becomes distended and makes a drumming sound when I tap on it.
Additionally, I have severe pain in my lower left abdomen that makes it difficult to stand.
Could you please tell me what kind of condition this might indicate?

Qingqing, 10~19 year old female. Ask Date: 2015/07/07

Hello, Miss: It is likely due to poor intestinal motility and excessive gas production after food digestion.
You should eat at regular intervals and in appropriate quantities, avoid gas-producing foods, and maintain regular bowel movements.
If necessary, you may visit a gastroenterology clinic for medication and stool examination.
Changhua Hospital cares about your health.

Left lower abdominal pain and bloating can be caused by a variety of gastrointestinal issues, and your symptoms suggest that there may be underlying conditions that need to be addressed. Based on your description, it seems that you have a history of gastrointestinal discomfort, including constipation and bloating, which can be indicative of several potential issues.

One common cause of left lower abdominal pain, especially in conjunction with bloating, is irritable bowel syndrome (IBS). IBS is a functional gastrointestinal disorder characterized by symptoms such as abdominal pain, bloating, gas, and changes in bowel habits (constipation or diarrhea). The pain often improves after a bowel movement, and dietary factors can significantly influence symptoms. Stress and anxiety can also exacerbate IBS symptoms.

Another possibility is diverticular disease, which includes diverticulosis and diverticulitis. Diverticulosis occurs when small pouches (diverticula) form in the walls of the colon, often due to a low-fiber diet. While many people with diverticulosis do not experience symptoms, some may develop diverticulitis, which is the inflammation or infection of these pouches. Symptoms of diverticulitis can include left lower abdominal pain, fever, and changes in bowel habits. If you have a history of constipation, this could increase the risk of developing diverticula.

Additionally, your symptoms could be related to gastrointestinal obstruction or functional dyspepsia. A blockage in the intestines can lead to severe pain, bloating, and an inability to pass gas or stool. Functional dyspepsia, on the other hand, is characterized by discomfort or pain in the upper abdomen, often associated with bloating and early satiety.

It's also important to consider the possibility of gynecological issues, especially if you are female. Conditions such as ovarian cysts, endometriosis, or pelvic inflammatory disease can cause left-sided abdominal pain and may be exacerbated by eating or certain activities.

Given the complexity of your symptoms, it is crucial to consult with a healthcare provider for a thorough evaluation. They may recommend diagnostic tests such as blood tests, imaging studies (like an ultrasound or CT scan), or endoscopy to better understand the underlying cause of your pain and bloating.
In the meantime, managing your diet may help alleviate some symptoms. Increasing fiber intake gradually can help with constipation, while keeping a food diary can help identify any specific triggers that worsen your bloating. Staying hydrated and engaging in regular physical activity can also promote healthy digestion.

In summary, left lower abdominal pain and bloating can stem from various gastrointestinal or gynecological issues. A comprehensive evaluation by a healthcare professional is essential to determine the exact cause and develop an appropriate treatment plan. If your symptoms persist or worsen, do not hesitate to seek medical attention.
