Irregular menstruation with decreased menstrual flow, characterized by thick blood mixed with a large amount of blood clots?

Hello Doctor: Seven years ago, I experienced abnormal menstruation, lower abdominal pain, and vaginal itching, and I underwent an abdominal ultrasound which revealed an ovarian cyst.
At that time, I had a habit of masturbation and was unaware that the cervix lacks pain receptors, which led to a laceration of the surrounding tissue and the expulsion of a fleshy mass.
Since there was no immediate bleeding, I did not pay much attention to it.
Subsequently, I often experienced severe abdominal pain, heavy bleeding, and irregular menstrual cycles.
Each time I menstruate, I have intense abdominal pain, pale complexion, and physical weakness.
Recently, my menstrual flow has decreased, and I am passing dark brown, thick menstrual blood along with large blood clots.
I also have abnormal foul-smelling discharge and itching, and my abdomen frequently feels swollen and uncomfortable.
Could this be cancer or other conditions? (P.S.
I have had sexual experiences.) I have been experiencing discomfort for a long time, and despite seeing doctors and taking medication, there has been no significant improvement.
I appreciate your response.
Thank you.

1.
Irregular menstruation requires a clinic visit to check for persistent ovarian cysts or other organic lesions.
2.
If there is frequent lower abdominal pain accompanied by foul-smelling or increased yellow-green discharge, it may indicate pelvic inflammatory disease.
It is recommended to seek treatment at a clinic and undergo a Pap smear examination.

Understanding abnormal menstrual symptoms is crucial for identifying potential underlying health issues. Based on your description, it seems you have been experiencing a range of concerning symptoms, including severe abdominal pain, irregular menstrual cycles, heavy bleeding, and unusual discharge. These symptoms can be indicative of various gynecological conditions, some of which may require immediate medical attention.

One possible cause of your symptoms could be the presence of ovarian cysts, which you mentioned were identified during a previous ultrasound. Ovarian cysts are fluid-filled sacs that can form on the ovaries and may cause pain, especially if they rupture or become large. They can also lead to irregular menstrual cycles and changes in bleeding patterns. However, the presence of deep brown, thick menstrual blood and large blood clots, along with foul-smelling discharge, raises concerns for other conditions.

Endometriosis is another condition that could explain your symptoms. This occurs when tissue similar to the lining of the uterus grows outside of it, leading to severe pain, especially during menstruation, and can cause heavy bleeding and clotting. The presence of abnormal discharge and itching may also suggest an infection or another gynecological issue, such as pelvic inflammatory disease (PID), which can occur due to sexually transmitted infections (STIs) or other factors.

Another serious concern is the possibility of uterine fibroids or even endometrial cancer, particularly given your family history and the prolonged duration of your symptoms. Uterine fibroids are non-cancerous growths in the uterus that can cause heavy bleeding, pain, and pressure symptoms. On the other hand, endometrial cancer can present with abnormal bleeding, especially in women with a history of irregular periods or other risk factors.

Given your symptoms and the potential for serious underlying conditions, it is essential to seek comprehensive medical evaluation. A thorough gynecological examination, including a pelvic ultrasound and possibly a hysteroscopy, may be necessary to visualize the uterus and ovaries better. Additionally, a biopsy may be warranted if there are concerns about abnormal tissue growth.

It is also important to address your mental and emotional health, especially if you have been experiencing anxiety related to your symptoms. Chronic stress and anxiety can exacerbate physical symptoms and may contribute to the perception of pain and discomfort. Engaging in stress-reducing activities, such as mindfulness, yoga, or counseling, may help improve your overall well-being.

In summary, your symptoms warrant further investigation to rule out serious conditions such as endometriosis, fibroids, or cancer. It is crucial to communicate openly with your healthcare provider about your concerns and the impact these symptoms have on your quality of life. Early diagnosis and appropriate management can significantly improve your health outcomes and alleviate your symptoms. Please prioritize scheduling a follow-up appointment with your gynecologist to discuss your symptoms in detail and explore the necessary diagnostic steps.
