Menstrual irregularities

Hello, doctor.
I am 22 years old and have been experiencing irregular menstruation for a long time.
I have seen both traditional Chinese medicine and Western medicine, but there hasn't been much improvement.
In March, I started seeing a Western doctor who diagnosed me with anovulatory cycles.
The doctor prescribed ovulation medication, and for the first three months, my menstrual cycles were normal while I tracked my basal body temperature daily.
However, this month my period has not arrived yet.
The last menstrual period was from June 3 to June 7, and there was some brown discharge between July 10 and July 14.
Since then, I still have not had my period.
Today, during my follow-up appointment, the doctor indicated that I am experiencing an anovulatory cycle this month.
However, after an ultrasound, he noted that the endometrial lining has not thickened, and after a pelvic exam, he changed his diagnosis to ovulatory bleeding.
He advised me to continue taking the ovulation medication, predicting that my period will arrive in two weeks.
What should I do if it doesn't come? Since July 10, my basal body temperature has been consistently low, ranging from 35 to 36 degrees Celsius.
I would like to ask the doctor if there is a possibility of pregnancy in my situation.
Am I experiencing anovulatory cycles? Will I face infertility in the future? Should I consider seeing a traditional Chinese medicine practitioner for treatment? Thank you for your help!

Hello, if you want to know if you are pregnant, you can take a pregnancy test.
If you had brown discharge between July 10 and July 14, which is considered your period, then your menstruation should not arrive so early two weeks later.
It seems that you may not be ovulating this month.
If it were me, I would first consider taking medication to induce menstruation before trying ovulation induction drugs.
However, I still recommend consulting your obstetrician-gynecologist.
Wishing you good health.

More Info

Irregular menstruation can be a complex issue, and it often requires a multifaceted approach to understand its underlying causes and to find effective treatments. Based on your description, it seems that you have been experiencing anovulatory cycles, which means that your ovaries are not releasing eggs regularly. This can lead to irregular periods, and it appears that you have already consulted both Western and Traditional Chinese Medicine (TCM) practitioners without significant improvement.

The fact that you were prescribed ovulation-inducing medication and experienced regular periods for a few months is a positive sign, indicating that the medication was effective in stimulating ovulation. However, the return of irregular cycles and the absence of menstruation this month, along with the low basal body temperature readings, raises some concerns.

Possible Causes of Irregular Menstruation
1. Hormonal Imbalances: Irregular menstruation is often linked to hormonal imbalances, particularly involving estrogen and progesterone. Conditions such as polycystic ovary syndrome (PCOS) can lead to anovulation and irregular cycles.

2. Stress and Lifestyle Factors: High levels of stress, significant weight changes, excessive exercise, or poor nutrition can also disrupt the menstrual cycle.

3. Thyroid Issues: Hypothyroidism or hyperthyroidism can affect menstrual regularity. Thyroid function tests may be warranted if not already done.

4. Underlying Health Conditions: Conditions such as endometriosis or uterine fibroids can also contribute to irregular menstruation and should be evaluated.

Concerns Regarding Pregnancy
Given that you have had irregular cycles and the recent absence of menstruation, it is essential to consider the possibility of pregnancy. If you are sexually active and there is a chance of conception, a home pregnancy test can provide clarity. If the test is negative and menstruation does not occur, further evaluation may be necessary.

Future Fertility Considerations
Your concern about potential infertility is valid. Anovulation can lead to difficulties in conceiving, but it does not mean that you will be infertile. Many women with irregular cycles can conceive with appropriate treatment. It is crucial to work closely with your healthcare provider to monitor your cycle and adjust treatment as needed.

Treatment Options
1. Continue with Ovulation Induction: If your doctor has suggested continuing with the ovulation medication, it may help regulate your cycles again. Regular monitoring through ultrasounds and blood tests can provide insight into your response to treatment.

2. Lifestyle Modifications: Incorporating stress-reducing activities, maintaining a balanced diet, and ensuring regular exercise can positively impact your menstrual health.

3. Consider TCM: If you feel that TCM may offer additional benefits, it could be worth exploring alongside your current treatment plan. Some women find that acupuncture and herbal remedies help regulate their cycles.

4. Further Evaluation: If irregularities persist, consider asking your doctor about additional tests, such as hormone level assessments, thyroid function tests, or imaging studies to rule out any anatomical issues.

Conclusion
In conclusion, while your current situation may feel concerning, there are various avenues to explore for treatment and management of your irregular menstruation. It is essential to maintain open communication with your healthcare provider, follow their recommendations, and advocate for further evaluation if necessary. Remember, many women experience irregular cycles at some point, and with the right support and treatment, it is possible to achieve regularity and address any fertility concerns.
