Rubber band ligation for internal hemorrhoids?

I underwent a second rubber band ligation for hemorrhoids, and about two days later, the rubber band fell off.
After it fell off, I noticed a significantly swollen mass protruding from the anus, which is even larger than before the ligation.
Does this indicate a failure of the procedure? It is currently very swollen.
Will it resolve on its own, or could this be a complication from the ligation? (The first ligation did not present these issues.) Thank you for your response.

Rubber band ligation (RBL) is a common procedure used to treat hemorrhoids, particularly internal hemorrhoids. While it is generally considered safe and effective, complications can arise, and your experience of significant swelling and protrusion after the procedure warrants attention.

Firstly, it is important to understand the mechanism of rubber band ligation. During the procedure, a rubber band is placed around the base of the hemorrhoid, cutting off its blood supply. Over time, the tissue dies and falls off, ideally leading to a reduction in the size of the hemorrhoid. However, complications can occur, including pain, bleeding, and, as you have experienced, significant swelling.

The swelling you are observing could be due to several factors:
1. Inflammatory Response: After the ligation, the body may respond with inflammation, leading to swelling. This is a normal part of the healing process, but if the swelling is excessive or accompanied by severe pain, it may indicate a complication.

2. Thrombosis: Sometimes, the ligation can lead to the formation of a blood clot within the hemorrhoid, resulting in a thrombosed hemorrhoid. This can cause significant swelling and pain. If the swelling is firm and painful, this could be a possibility.

3. Infection: Although rare, an infection can occur at the site of ligation, leading to increased swelling and possibly discharge. Signs of infection include fever, increased pain, and redness around the area.

4. Incomplete Ligation: If the rubber band fell off prematurely, it may indicate that the ligation was not successful. This could lead to the hemorrhoid remaining engorged and swollen.

Given that you did not experience these symptoms after your first ligation, it is essential to consult your healthcare provider. They will likely perform a physical examination to assess the area and determine the cause of the swelling. They may also consider imaging studies if they suspect complications that are not visible externally.

In terms of management, if the swelling is due to inflammation, your doctor may recommend conservative measures such as sitz baths, topical analgesics, and anti-inflammatory medications. If a thrombosed hemorrhoid is diagnosed, treatment options may include conservative management or, in some cases, surgical intervention to remove the clot.

It is also crucial to monitor for any signs of complications, such as persistent bleeding, severe pain, or fever, which would necessitate immediate medical attention.

In conclusion, while some swelling can be expected after rubber band ligation, significant swelling, especially if it is more pronounced than before the procedure, should be evaluated by your healthcare provider. They will be able to determine whether this is a normal part of the healing process or if it indicates a complication that requires further intervention. Always prioritize open communication with your healthcare team to ensure the best outcomes following any medical procedure.
