Is the likelihood of getting pregnant immediately after an abortion high?

Hello doctor, I would like to inquire about something.
I had a vacuum aspiration on July 3rd and followed the post-procedure care for two weeks.
On July 24th, I had sexual relations with my boyfriend despite my better judgment.
To be safe, I first helped him with manual stimulation twice, and then we used a condom for the entire duration until he ejaculated in the condom.
The next morning, we had intercourse again, and we started with a condom, but it became dry, so I removed it, and he entered me without a condom for less than a minute before he ejaculated externally.
My period is expected around August 3rd, but I am worried that it might not come.
I am concerned about the possibility of pregnancy from that brief moment.
Is there a chance I could be pregnant?

Xiao Qin Qin, 20~29 year old female. Ask Date: 2015/07/26

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, the chances of pregnancy are low, but it's still best to use contraception.
Wishing you good health.

After an abortion, it is indeed possible to become pregnant again quite quickly. The body can return to its normal ovulation cycle within a few weeks, which means that if you engage in unprotected intercourse during this time, there is a risk of conception. In your case, since you had a vacuum aspiration abortion on July 3rd and then had sexual intercourse on July 24th, you could potentially have been in your fertile window, depending on your individual cycle.

To address your concerns about the risk of pregnancy from the encounters you described, it’s important to consider a few factors:
1. Timing of Ovulation: After an abortion, ovulation can occur as soon as two weeks later. If you had unprotected intercourse during your fertile window, there is a chance of becoming pregnant. Sperm can live inside the female reproductive tract for up to five days, so if you ovulated shortly after your sexual activity, conception could occur.

2. Contraceptive Methods: You mentioned using condoms during intercourse, which is a reliable method of contraception when used correctly. However, if the condom was dry and you removed it before your partner ejaculated, there is a risk of pre-ejaculate fluid containing sperm coming into contact with the vaginal area. While the risk is lower than with unprotected intercourse, it is not zero.

3. Menstrual Cycle: You indicated that you expect your period around August 3rd. If your cycle is regular, you can use this as an indicator of whether you might be pregnant. A missed period is often one of the first signs of pregnancy. If your period does not arrive on time, it would be advisable to take a home pregnancy test for confirmation.

4. Health Considerations: After an abortion, it is crucial to allow your body time to heal. Engaging in sexual activity too soon can increase the risk of complications, such as infection. It is generally recommended to wait at least two weeks after an abortion before resuming sexual intercourse, but this can vary based on individual circumstances and the advice of your healthcare provider.

5. Emotional and Psychological Factors: It’s also important to consider the emotional aspects of engaging in sexual activity after an abortion. Many individuals experience a range of emotions following the procedure, and it’s essential to ensure that both partners are comfortable and consenting to sexual activity.

In summary, while it is possible to become pregnant shortly after an abortion, the use of condoms significantly reduces that risk. If you are concerned about the possibility of pregnancy, monitoring your menstrual cycle and taking a pregnancy test if your period is late are good steps to take. Additionally, if you have any further concerns or experience any unusual symptoms, it is advisable to consult with a healthcare provider for personalized advice and support.
