Genital issues
Hello doctor, I would like to ask about a situation I've been experiencing.
In the winter, I noticed that my genitalia would sometimes shrink to about 3-4 centimeters when not erect, and I thought it was due to the cold weather.
However, now that it's summer, I find that during activities like running, push-ups, or lifting heavy objects, my foreskin retracts and it feels uncomfortable, almost like phimosis, also to about 3-4 centimeters.
Sometimes it even gets caught in hair.
My erections are normal, but since I will be enlisting in the military soon, I am concerned about the long-term effects of my penis being covered like this.
Will this be a problem? Is there a treatment for this?
Niming, 20~29 year old female. Ask Date: 2015/07/27
Dr. Fu Shizhang reply Urology
It is recommended to visit an outpatient clinic for examination of phimosis issues.

Penile shrinkage or retraction, particularly in the context of physical activity or temperature changes, can be a concerning issue for many men. It is important to understand that the penis can naturally retract or appear smaller due to various factors, including temperature, physical exertion, and even anxiety. In colder temperatures, the body instinctively conserves heat by constricting blood vessels, which can lead to a temporary reduction in size. Similarly, during physical activities like running or push-ups, the body may respond to exertion by tightening the muscles in the pelvic area, which can also cause the penis to retract.
From your description, it seems that you are experiencing this retraction more frequently, particularly during physical activities, and that it is accompanied by discomfort. The sensation of the foreskin (if you are uncircumcised) pulling back or feeling tight can lead to discomfort, especially if it feels like it is constricting the penis. This can sometimes be mistaken for phimosis, a condition where the foreskin cannot be easily retracted over the glans (the head of the penis).
While it is good to hear that you are experiencing normal erections, the discomfort you are describing should not be ignored. Prolonged retraction or constriction can potentially lead to issues such as skin irritation, inflammation, or even pain during erections. If the foreskin is too tight, it can lead to complications, including infections or scarring.
Here are some considerations and potential treatment options:
1. Consult a Healthcare Provider: It is essential to discuss your symptoms with a healthcare provider, preferably a urologist. They can perform a physical examination and determine if there are any underlying conditions that need to be addressed.
2. Hygiene and Care: Maintaining good hygiene is crucial, especially if you are experiencing tightness or discomfort. Ensure that the area is clean and dry to prevent irritation.
3. Gentle Stretching: If you are diagnosed with phimosis or tight foreskin, your doctor may recommend gentle stretching exercises or the use of topical corticosteroids to help loosen the skin.
4. Avoiding Irritants: Be mindful of any products that may irritate the genital area, including soaps or lotions that are heavily scented or contain harsh chemicals.
5. Physical Activity Adjustments: If certain activities exacerbate the issue, consider modifying your routine. Wearing supportive underwear during exercise can help minimize discomfort.
6. Surgical Options: In cases where phimosis is severe and causing significant issues, surgical options such as circumcision or preputioplasty may be considered.
7. Monitoring Changes: Keep track of any changes in symptoms, including frequency, duration, and any additional discomfort. This information can be helpful for your healthcare provider.
In summary, while temporary shrinkage of the penis can be normal due to various factors, persistent discomfort or changes in size during physical activities warrant a professional evaluation. Early intervention can help prevent complications and ensure that you are comfortable, especially as you prepare for military service. Remember, addressing these concerns with a healthcare professional is the best way to find an appropriate solution tailored to your specific situation.
