Facial asymmetry and small eyes issues..?

Hello doctor, I have a question.
My face is asymmetrical; the right side of my face droops, and when I smile, my mouth tilts to one side.
This is especially noticeable when I am tired.
What could be the issue? I had uneven eyes when I was younger, but now my facial asymmetry has become more pronounced.
Which specialty should I consult for this?

Nanbu Ren, 20~29 year old female. Ask Date: 2015/07/27

Hello: Facial asymmetry occurs in most people, but it is usually very mild and not noticeable.
It often results from dental occlusion issues, where the same side of the teeth is frequently used, leading to uneven blood flow in the face and increasing differences in size over time.
You should consult a maxillofacial surgeon.

More Info

Facial asymmetry, particularly when it becomes pronounced or changes over time, can be a source of concern for many individuals. It is important to understand that some degree of asymmetry is normal in human faces; however, significant changes or noticeable asymmetry can indicate underlying health issues that may require medical attention.

Causes of Facial Asymmetry
1. Genetic Factors: Many people have naturally asymmetrical features due to genetic predisposition. This can include variations in the size and shape of facial structures.

2. Developmental Issues: Conditions such as hemifacial microsomia or other congenital syndromes can lead to asymmetry as the face develops.

3. Neurological Conditions: Facial asymmetry can be a sign of neurological issues, such as Bell's palsy, which causes temporary weakness or paralysis of the facial muscles on one side of the face. Other neurological disorders, including stroke or transient ischemic attacks (TIAs), can also result in facial drooping.

4. Muscle Weakness: Conditions that affect the muscles of the face, such as myasthenia gravis, can lead to asymmetry, particularly when smiling or making facial expressions.

5. Injury or Trauma: Past injuries to the face or jaw can result in changes to the structure of the face, leading to asymmetry.

6. Dental Issues: Misalignment of teeth or jaw problems (such as temporomandibular joint disorders) can also contribute to facial asymmetry.

7. Aging: As we age, the skin loses elasticity, and the underlying structures of the face can change, leading to increased asymmetry.

When to Seek Help
If you notice that your facial asymmetry has become more pronounced, especially if accompanied by other symptoms such as weakness, numbness, or difficulty speaking, it is crucial to seek medical attention promptly. Here are some signs that indicate you should consult a healthcare professional:
- Sudden Changes: If the asymmetry appears suddenly, particularly if it is accompanied by other neurological symptoms (e.g., weakness in the arms or legs, difficulty speaking, or severe headaches), you should seek emergency medical care.

- Persistent Symptoms: If you have ongoing issues with facial drooping, difficulty smiling, or any other functional impairment, it is advisable to consult a doctor.

- Pain or Discomfort: If you experience pain associated with the asymmetry, such as jaw pain or headaches, this should also be evaluated.

Which Specialist to Consult
For facial asymmetry, you may want to start by consulting a primary care physician who can assess your symptoms and refer you to the appropriate specialist. Depending on the underlying cause, you may be referred to:
- Neurologist: If there are concerns about neurological conditions.

- Otolaryngologist (ENT): For issues related to the ear, nose, and throat that may affect facial structure.

- Plastic Surgeon: If you are considering surgical options for cosmetic correction.

- Dentist or Orthodontist: For dental-related issues that may be contributing to asymmetry.

Conclusion
Facial asymmetry can arise from various causes, ranging from benign genetic factors to more serious medical conditions. If you are experiencing noticeable changes in your facial symmetry, especially if accompanied by other symptoms, it is important to seek medical advice. Early intervention can help address any underlying issues and provide you with options for treatment or management. Remember, your health and well-being are paramount, and seeking help is a positive step toward understanding and addressing your concerns.
