Anal pain and burning sensation?

Hello, Dr.
Ke.
A few weeks ago, I experienced diarrhea, and a few days later, I developed itching around the anus, along with severe pain and a burning sensation.
I applied Mentholatum, which provided some relief! However, I feel more pain after each bowel movement.
A few days ago, I increased my water intake and fiber-rich foods, which initially helped, but today after using the restroom, the pain returned, and I noticed a bit of blood.
Could this be a minor anal fissure caused by the diarrhea?

Mr. Hsu, 20~29 year old female. Ask Date: 2015/07/29

Dr. Ke Fangxu reply Surgery

Based on your description, it seems that there is a higher likelihood of a minor anal fissure caused by diarrhea.
It is recommended to take warm sitz baths to help improve the condition.
Mentholatum is not advised due to its menthol content.
You can purchase hemorrhoid ointment at a pharmacy for use.
If symptoms do not improve, please consult a physician for further evaluation.

Experiencing anal pain and a burning sensation after diarrhea can be quite distressing, and it's important to understand the underlying causes and potential treatments. Based on your description, it seems that your symptoms may be related to irritation or injury in the anal area, which can occur after episodes of diarrhea.

Diarrhea can lead to several complications in the anal region. The frequent passage of loose stools can cause irritation to the sensitive skin around the anus, leading to symptoms such as itching, burning, and pain. This irritation can be exacerbated by wiping, especially if rough toilet paper is used or if the area is not kept clean and dry. In some cases, this irritation can progress to a condition known as anal fissures, which are small tears in the lining of the anus. These fissures can cause sharp pain, especially during bowel movements, and may also result in bleeding.

The presence of blood, as you mentioned, is a concerning symptom that should not be overlooked. While minor bleeding can occur with anal fissures, it is essential to rule out other potential causes, such as hemorrhoids, infections, or more serious conditions like inflammatory bowel disease or colorectal issues. Given that you have experienced significant pain and bleeding, it would be advisable to consult a healthcare professional for a thorough evaluation.

In the meantime, there are several strategies you can employ to alleviate your symptoms:
1. Maintain Hygiene: Keep the anal area clean and dry. After bowel movements, consider using gentle wipes or a bidet instead of toilet paper to minimize irritation.

2. Topical Treatments: Over-the-counter creams or ointments, such as hydrocortisone or barrier creams, can help soothe irritation. You mentioned using Mentholatum, which may provide temporary relief due to its cooling effect, but it’s essential to ensure that it does not further irritate the area.

3. Dietary Adjustments: Continue to consume a high-fiber diet and drink plenty of water to maintain soft stools. This can help reduce the frequency and severity of diarrhea, which in turn may alleviate the irritation.

4. Warm Baths: Soaking in a warm bath can help relax the anal muscles and provide relief from pain and discomfort.

5. Avoid Straining: If you experience constipation, avoid straining during bowel movements, as this can worsen anal fissures and pain.

6. Pain Relief: Over-the-counter pain relievers, such as acetaminophen or ibuprofen, can help manage pain and discomfort.

If your symptoms persist or worsen, or if you notice significant bleeding, it is crucial to seek medical attention. A healthcare provider may perform a physical examination and possibly recommend further testing, such as a colonoscopy, to rule out any underlying conditions. They may also prescribe medications or treatments tailored to your specific situation.

In summary, while your symptoms may be related to irritation from diarrhea, the presence of blood and significant pain warrants a professional evaluation. Taking steps to maintain hygiene, using topical treatments, and making dietary adjustments can help manage your symptoms, but do not hesitate to reach out to a healthcare provider for further guidance.
