Diarrhea
Hello Dr.
Hong: I have been experiencing diarrhea for four days, with about five bowel movements each day.
I feel gas moving in my abdomen (two years ago, I had an endoscopy that showed some erosion, and I often experience bloating in my stomach).
Recently, I have been under a lot of stress and feeling anxious (the anxiety stems from having had high-risk sexual behavior four months ago; I have tested for HIV a total of 12 times in the past four months and am relieved that all results have been negative, but I have still felt anxious for these four months).
My bowel movements have been irregular until I started having diarrhea four days ago.
My overall energy is fairly good, but I occasionally have headaches.
Could you please help me understand what might be causing my diarrhea? Thank you.
P.S.
I have had my spleen removed.

Based on your description, diarrhea may be triggered by emotions or certain foods (such as dairy products), which are typical clinical manifestations of irritable bowel syndrome (IBS).
It is recommended to maintain a calm mood and avoid foods that cause gastrointestinal discomfort.
If further diagnosis and treatment are needed, please visit a gastroenterology specialist for a thorough examination.

Hello! It sounds like you're experiencing a challenging situation with your gastrointestinal health, particularly with diarrhea lasting for four days and accompanied by feelings of gas and bloating. Given your medical history, including a previous gastric issue and recent stressors, let's delve into potential causes and concerns related to your symptoms.
Diarrhea can arise from various factors, including infections, dietary changes, stress, and underlying medical conditions. In your case, the stress you've mentioned could be a significant contributor. Stress can lead to gastrointestinal disturbances, often referred to as "nervous stomach" or "stress-induced diarrhea." The gut-brain connection is well established; when you're anxious or stressed, your body can react by altering gut motility, leading to increased bowel movements and diarrhea.
Since you mentioned a history of gastric issues, including a previous gastric ulcer, it's essential to consider that your gastrointestinal tract may be more sensitive to stress and dietary changes. Additionally, the fact that you have had your spleen removed (splenectomy) can also impact your immune response and how your body handles infections, making you more susceptible to gastrointestinal disturbances.
The frequency of your bowel movements (approximately five times a day) and the sensation of gas could indicate that your body is reacting to something specific, such as a food intolerance or an infection. If you have recently changed your diet or consumed something that didn't agree with you, that could also be a factor.
It's also worth noting that anxiety and stress can lead to tension in the abdominal area, which may contribute to feelings of bloating and discomfort. The headaches you mentioned could be tension-related, stemming from the stress you're experiencing.
Given that your diarrhea has persisted for four days, it's essential to monitor your symptoms closely. Here are some recommendations:
1. Hydration: Ensure you stay well-hydrated, as diarrhea can lead to dehydration. Drinking clear fluids, such as water, broth, or electrolyte solutions, can help replenish lost fluids.
2. Diet: Consider following a bland diet (BRAT diet: bananas, rice, applesauce, toast) to help ease your digestive system. Avoid dairy, fatty foods, and high-fiber foods until your symptoms improve.
3. Stress Management: Since stress appears to be a significant factor, incorporating stress-reduction techniques such as mindfulness, meditation, or gentle exercise may help alleviate some of your symptoms.
4. Medical Evaluation: If your diarrhea persists beyond a week, or if you experience additional symptoms such as fever, severe abdominal pain, or blood in your stool, it would be prudent to seek medical attention. A healthcare provider can assess for potential infections, inflammatory bowel disease, or other underlying conditions.
5. Follow-Up on Anxiety: Given your history of anxiety related to your previous risk behavior, it might be beneficial to discuss these feelings with a mental health professional. They can provide support and coping strategies to help manage your anxiety, which may, in turn, alleviate some of your gastrointestinal symptoms.
In summary, while stress and anxiety may be contributing to your diarrhea, it's essential to monitor your symptoms and seek medical advice if they persist or worsen. Taking care of your mental health is just as important as addressing your physical symptoms. I hope you find relief soon!
