Frequent Rectal Bleeding: When to Seek Medical Attention - Surgery

Blood in the stool multiple times?

Hello, Dr.
Ke: In the past seven days, I have experienced five consecutive episodes of rectal bleeding, and the blood has been bright red each time.
The amount of blood is significant enough to soak through the toilet paper.
The last three episodes included one instance of soft stool and two instances of diarrhea, with intervals of about 10-20 minutes, and the blood was enough to penetrate two layers of toilet paper.
However, the more I wipe, the less blood appears on the tissue.
I would like to know if this could be due to internal hemorrhoids, polyps, or if there is a risk of any intestinal cancer.
Do I need to return to my home country for a colonoscopy, as I am currently studying abroad? Or should I observe for a few more days and adjust my diet to lighter foods before deciding whether to purchase a plane ticket? Thank you for your assistance, Dr.
Ke.

Based on your description, it seems that the likelihood of hemorrhoidal bleeding is higher; however, the possibility of polyps and cancer is not zero.
If the bleeding gradually decreases, there is no need for concern.
However, if the bleeding is severe and does not improve, it would be better to undergo further examination.

Frequent rectal bleeding, especially when accompanied by changes in bowel habits such as diarrhea or soft stools, can be concerning and warrants prompt medical attention. The fact that you have experienced bright red blood during bowel movements on multiple occasions raises several possibilities, including hemorrhoids, anal fissures, diverticular disease, polyps, or even colorectal cancer.
1. Potential Causes:
- Hemorrhoids: These are swollen blood vessels in the rectal area that can bleed, especially during bowel movements. They often present with bright red blood on toilet paper or in the toilet bowl.

- Anal Fissures: Small tears in the lining of the anus can cause sharp pain and bleeding during bowel movements, often associated with hard stools.

- Polyps: These are growths on the lining of the colon or rectum that can bleed. While many polyps are benign, some can develop into cancer over time.

- Diverticular Disease: Diverticula are small pouches that can form in the walls of the colon. If they become inflamed or infected, they can cause bleeding.

- Colorectal Cancer: Although less common in younger individuals, any persistent rectal bleeding should be evaluated to rule out malignancy.

2. When to Seek Medical Attention:
Given that you have had five episodes of rectal bleeding in the past week, it is advisable to seek medical attention rather than waiting. While dietary changes may help alleviate symptoms if they are related to hemorrhoids or minor issues, the frequency and volume of blood you describe are concerning. It is important to rule out more serious conditions, especially if you have a family history of colorectal cancer or other gastrointestinal diseases.

3. Diagnostic Procedures:
A colonoscopy is the gold standard for evaluating the lower gastrointestinal tract. It allows for direct visualization of the colon and rectum and can help identify the source of bleeding. If you are currently abroad, you should consider scheduling a colonoscopy as soon as possible, as this will provide clarity on your condition. If travel is not feasible immediately, consult with a local healthcare provider who can evaluate your symptoms and possibly perform preliminary tests.

4. Dietary Considerations:
While waiting for medical evaluation, you may benefit from a diet high in fiber to help regulate bowel movements and prevent straining, which can exacerbate bleeding from hemorrhoids or fissures. Staying hydrated is also crucial. However, dietary changes should not replace the need for medical evaluation.

5. Conclusion:
In summary, while there are several potential benign causes for your symptoms, the frequency and nature of your rectal bleeding necessitate a thorough evaluation. It is crucial to consult a healthcare provider as soon as possible to determine the cause of your symptoms and to initiate appropriate management. Delaying evaluation could lead to complications, especially if a more serious condition is present. Your health is paramount, and timely intervention is key to addressing these symptoms effectively.
