Menstrual delay
Hello, my girlfriend has irregular menstrual cycles.
Her period was expected on July 22, but as of today (August 7), it is 15 days late.
During the week of July 22, she experienced typical premenstrual symptoms (slight fever, cold, discomfort in the lower abdomen), but her period did not arrive.
She mentioned feeling as if the blood is not flowing out.
Since her last period ended on July 22, there were only 1-2 instances of intercourse (around June 26, 3-4 days after her last period), and during those times, condoms were used without any breakage or leakage.
We also took a pregnancy test last week (August 1), which showed no signs of pregnancy.
Additionally, my girlfriend has not experienced any other symptoms, only slight fatigue.
The unusual aspect is that she has noticed less vaginal lubrication before the recent instances of intercourse, which has led to a decrease in sexual activity.
Could she be pregnant, or what could be causing the delay in her period?

Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, based on your description, there is a possibility of pregnancy.
It is advisable for you to consult a physician to arrange for tests to determine if you are pregnant or to investigate the causes of irregular menstruation, in order to avoid encountering this issue frequently in the future.
Besides pregnancy, irregular ovulation, polycystic ovary syndrome, uterine or ovarian lesions, brain lesions, lifestyle factors, emotional state, diet, and age can also contribute to delayed menstruation.
Thank you! Wishing you peace and health, Dr.
Chang.

Delayed menstruation can be a source of concern for many women, especially when it deviates from their regular cycle. In your girlfriend's case, her menstrual cycle has been irregular, with her last period on July 22 and a delay of 15 days as of August 7. Understanding the potential causes of this delay is crucial for addressing any concerns and determining the next steps.
Firstly, it is essential to consider the possibility of pregnancy. Although your girlfriend has taken a pregnancy test on August 1 with negative results, it is important to note that pregnancy tests are most accurate when taken after a missed period. If there is still concern about pregnancy, it may be advisable to retest after a few days or consult a healthcare provider for a blood test, which can detect pregnancy earlier than a urine test.
If pregnancy is ruled out, several other factors could contribute to the delay in menstruation. Stress is a significant factor that can disrupt the hormonal balance necessary for regular ovulation and menstruation. Emotional stress, physical stress, or significant life changes can lead to hormonal fluctuations, resulting in delayed or missed periods. Additionally, lifestyle factors such as changes in diet, exercise, and sleep patterns can also impact menstrual regularity.
Another potential cause of delayed menstruation is hormonal imbalances. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or other endocrine issues can lead to irregular cycles. Given that your girlfriend has experienced a sensation of blood not flowing out, it may indicate a possible hormonal issue or even a condition like anovulation, where the ovaries do not release an egg during the menstrual cycle.
Moreover, the use of hormonal contraceptives or medications can also affect menstrual cycles. If your girlfriend has been on any form of hormonal birth control, it could lead to changes in her cycle, including missed periods. Additionally, significant weight changes, whether loss or gain, can also impact menstrual regularity.
In your girlfriend's case, the recent experience of feeling fatigued and having less vaginal lubrication could also be related to hormonal changes. Low estrogen levels can lead to reduced vaginal secretions and may also contribute to feelings of fatigue. If these symptoms persist, it may be beneficial for her to consult a healthcare provider for a thorough evaluation.
In summary, while pregnancy is a primary concern with delayed menstruation, other factors such as stress, hormonal imbalances, lifestyle changes, and potential underlying health conditions should also be considered. If the delay continues or if any concerning symptoms arise, it is advisable for your girlfriend to seek medical advice. A healthcare provider can perform necessary tests, including blood work, to determine the underlying cause of her delayed menstruation and recommend appropriate treatment or management strategies.
