My ear has been feeling strange and very painful lately?
Hello doctor, initially my right ear was just feeling stuffy and itchy, so I kept using a cotton swab to clean it.
However, there were a few times when I ended up causing bleeding.
Gradually, it became more uncomfortable, and now it feels like tinnitus; it still itches, and when I try to clean it, there is fluid, and the material I remove feels sticky, almost like something is wet.
Now it has become very painful, and it feels stuffy like tinnitus.
What should I do?

Hello: It is possible that you have otitis externa.
If you are experiencing significant pain and a feeling of fullness, there may already be a bacterial infection.
It is advisable to seek medical evaluation and receive appropriate medication treatment.
Wishing you safety and health.
Sincerely, Dr.

More Info
Ear pain and discomfort can arise from various underlying conditions, and your symptoms suggest a few potential issues that may need to be addressed. Based on your description, it seems you are experiencing a combination of earwax buildup, possible external ear canal inflammation (external otitis), and possibly middle ear involvement.
Initially, you mentioned that your right ear felt itchy and you resorted to using cotton swabs to clean it. This is a common practice, but it can often lead to complications. Inserting objects into the ear canal can push earwax deeper, potentially causing impaction, or even injuring the delicate skin of the ear canal, leading to inflammation or infection. The bleeding you experienced is a sign that the skin may have been damaged, which can further exacerbate the situation.
The symptoms of ear fullness, pain, and the sensation of fluid or "water" in the ear can indicate several things. One possibility is that you have developed a condition known as cerumen impaction, where excessive earwax blocks the ear canal, leading to discomfort and hearing changes. The sticky, wet consistency of the discharge you described could be due to a combination of earwax and moisture, which can occur if there is an underlying infection or inflammation.
Another potential issue is external otitis, commonly known as swimmer's ear, which is an infection of the outer ear canal. This condition can cause pain, itching, and discharge, and is often exacerbated by moisture in the ear. If the ear canal is inflamed, it can also lead to a sensation of fullness or pressure, similar to what you are experiencing.
In some cases, if the inflammation or infection progresses, it can affect the middle ear, leading to a condition known as otitis media. This can cause more significant pain, pressure, and even hearing loss. Given that you are experiencing pain and a sensation of fullness, it is crucial to seek medical attention.
Here are some steps you can take to address your symptoms:
1. Avoid Further Irritation: Stop using cotton swabs or any other objects to clean your ears. This can worsen the situation by pushing wax deeper or causing injury.
2. Consult a Healthcare Professional: It is essential to see an ear, nose, and throat (ENT) specialist or a primary care physician who can examine your ear properly. They may perform an otoscopic examination to assess the condition of your ear canal and eardrum.
3. Possible Treatments: Depending on the diagnosis, treatment may include:
- Earwax Removal: If cerumen impaction is confirmed, a healthcare provider can safely remove the wax using specialized tools or irrigation.
- Medications: If an infection is present, antibiotic ear drops may be prescribed. For inflammation, corticosteroid drops may also be indicated.
- Pain Management: Over-the-counter pain relievers can help alleviate discomfort while you await treatment.
4. Follow-Up Care: If you have a history of recurrent ear problems, regular check-ups with an ENT specialist may be beneficial to monitor your ear health.
In conclusion, while ear pain and discomfort can stem from various causes, your symptoms suggest a need for professional evaluation and treatment. Prompt attention can help prevent complications and restore your ear health, allowing you to avoid further discomfort and potential hearing issues.
