Is it a prostate issue?

Hello doctor, recently I've noticed that when watching adult films, my erection lasts longer, and I experience a slight burning sensation and redness at the urethral opening.
When I'm not erect, the condition seems normal.
I would like to ask what issues might be occurring in my urinary system, as this situation has been ongoing for some time.
Thank you!

Dr. Lin Wenxu reply Urology

I apologize, but the change in the color of the glans may be due to engorgement of the penis during an erection.
The discomfort at the urethral opening could be caused by excessive tension in the pelvic floor muscles.
I cannot provide a definitive answer, so I recommend that you visit a urology clinic for further examination.

It sounds like you are experiencing a combination of symptoms related to your urinary and reproductive health, particularly concerning painful urination and discomfort during erections. Given your history of prostatitis and ongoing symptoms, it is essential to consider several potential underlying issues.

Firstly, the pain you are experiencing at the urethral opening, especially during erections or after prolonged arousal, could be indicative of a few conditions. One possibility is that you may be dealing with chronic prostatitis or chronic pelvic pain syndrome (CP/CPPS), which can manifest as pain in the pelvic region, including the urethra, perineum, and even the penis. This condition can be challenging to diagnose and treat, as it may not always involve a bacterial infection, and symptoms can vary widely among individuals.

Another potential issue could be related to urethral irritation or inflammation, which can occur due to various factors, including sexually transmitted infections (STIs), trauma, or even irritation from soaps or personal care products. If you have had unprotected sexual encounters, it is crucial to discuss this with your healthcare provider, as certain infections can lead to symptoms like yours.

The redness and stinging sensation at the urethral opening could also suggest a condition known as urethritis, which is inflammation of the urethra. This can be caused by infections, but it can also occur due to non-infectious factors, such as chemical irritants or trauma.
In terms of management, it is essential to maintain open communication with your healthcare provider. They may recommend a thorough evaluation, including urine tests, STI screenings, and possibly imaging studies, to rule out any significant underlying conditions. If an infection is suspected, appropriate antibiotic therapy may be initiated.
Additionally, lifestyle modifications can play a significant role in managing symptoms. Staying hydrated, avoiding irritants such as caffeine, alcohol, and spicy foods, and practicing safe sex can help reduce the risk of exacerbating your symptoms. Regular follow-ups with your urologist are also crucial, as they can monitor your condition and adjust treatment as necessary.

If your symptoms persist or worsen, further interventions may be needed. This could include physical therapy for pelvic floor dysfunction, medications to manage pain and inflammation, or even more advanced treatments depending on the underlying cause.

In summary, while your symptoms may be concerning, they are not uncommon in individuals with a history of prostatitis or pelvic pain syndromes. It is vital to seek medical advice to ensure a proper diagnosis and treatment plan tailored to your specific needs. Remember, early intervention can often lead to better outcomes and improved quality of life.
