Discomfort and swelling in the right testicle?

Dear Doctor: Since the end of June, I have been experiencing a dull discomfort in my right testicle after sitting for a period of time during class.
Sometimes, this is accompanied by a dull pain in my lower abdomen.
Up until now, my testicle still occasionally feels uncomfortable.
When I touch it, it doesn't feel as relaxed as the left one.
I also feel that my sperm volume has decreased compared to before.
This issue has been quite troubling for me, and I am unsure if it could be related to any medical condition.
Thank you!

Xingren, 10~19 year old female. Ask Date: 2015/08/11

Hello, you may be experiencing testicular inflammation or other conditions.
It could also be due to wearing tight pants or poor sitting posture, which may be compressing the right-side nerve.
In any case, it would be safer to consult a physician.
If you have concerns, you can visit a nearby medical facility for relevant tests.
If there are any unclear points, feel free to ask again for further discussion.
Online responses should only be considered as references; if necessary, you should seek medical attention at a healthcare facility.

Discomfort in the right testicle can be concerning and may arise from various causes. It’s essential to understand the potential reasons behind this discomfort and when to seek medical help.

Possible Causes of Right Testicle Discomfort
1. Epididymitis: This is an inflammation of the epididymis, the tube that stores and carries sperm. It can cause pain and swelling in the affected testicle, often accompanied by discomfort in the lower abdomen. It may be caused by infections, including sexually transmitted infections (STIs) or urinary tract infections (UTIs).

2. Orchitis: This is an inflammation of one or both testicles, which can occur due to viral or bacterial infections. Mumps is a common viral cause, especially if the individual has not been vaccinated.

3. Testicular Torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off blood supply to the testicle. Symptoms include sudden, severe pain, swelling, and nausea. Immediate medical attention is crucial to save the testicle.

4. Hernia: An inguinal hernia occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles. This can cause discomfort in the groin and testicle area, especially when sitting for prolonged periods.

5. Varicocele: This is an enlargement of the veins within the scrotum, similar to varicose veins. It can cause a dull ache or discomfort, particularly after standing or sitting for long periods.

6. Hydrocele: This is a fluid-filled sac surrounding a testicle, leading to swelling and discomfort. It is usually painless but can cause a feeling of heaviness.

7. Trauma or Injury: Any recent injury to the groin area can lead to discomfort in the testicle.
8. Tumors: Although less common, testicular tumors can cause discomfort or changes in the testicle.

When to Seek Medical Help
It is crucial to seek medical attention if you experience any of the following symptoms:
- Severe Pain: If the discomfort escalates to severe pain, especially if sudden, it could indicate a serious condition like testicular torsion.

- Swelling: Noticeable swelling of the testicle or scrotum should be evaluated.

- Changes in Size or Shape: Any significant changes in the size or shape of the testicle, or if one testicle feels harder than the other.

- Fever: Accompanying fever could indicate an infection.

- Nausea or Vomiting: These symptoms, especially when combined with testicular pain, warrant immediate medical evaluation.

- Changes in Urination: Pain during urination or changes in urinary habits can indicate an underlying issue.

Conclusion
Given your symptoms of discomfort in the right testicle, a feeling of heaviness, and a perceived decrease in sperm volume, it is advisable to consult a healthcare professional. They may perform a physical examination, and possibly an ultrasound, to determine the underlying cause of your discomfort. Early diagnosis and treatment are crucial to address any potential issues effectively. Remember, it’s always better to err on the side of caution when it comes to your health.
