Abnormal mammography results?

Hello Dr.
Zhou, I recently had a mammogram, and the results showed a shadow the size of a one-dollar coin.
Following the doctor's recommendation, I was referred to a breast specialty clinic for an ultrasound examination.
However, the ultrasound results indicated that both breasts were normal.
I would like to know if I need to undergo any further tests.
Additionally, I would like to ask if having the mammogram and ultrasound during my menstrual cycle could affect the results.
Thank you!

Xiao Wen, 40~49 year old female. Ask Date: 2015/08/26

Dr. Zhou Jiazhen reply Breast and Thyroid

Dear Xiaowen,
Hello.

1.
During the menstrual cycle, hormonal fluctuations may cause hyperplasia of the mammary ducts or breast pain, which can sometimes affect imaging and ultrasound procedures, leading to some discrepancies.

2.
What is the report from the imaging? If the ultrasound shows no abnormalities, a follow-up examination in about 3-6 months is recommended.
Wishing you peace and health.
Sincerely,
Zhou Jiazhen

Dear Patient,
Thank you for reaching out with your concerns regarding your recent mammogram and ultrasound results. It's understandable to feel anxious when faced with abnormal findings, but I hope to provide you with clarity on the next steps and address your questions.

Firstly, the mammogram indicated a shadow the size of a coin, which prompted your physician to refer you for an ultrasound. The ultrasound results showing no abnormalities in both breasts are reassuring. Generally, if a mammogram reveals a suspicious area but subsequent imaging (like an ultrasound) shows no abnormalities, it is often a good sign. However, the decision on whether further evaluation is needed depends on several factors, including your personal and family medical history, as well as the characteristics of the mammographic finding.

In many cases, if the ultrasound is clear and there are no other concerning symptoms (such as a palpable lump, changes in the skin, or discharge from the nipple), your doctor may recommend routine follow-up with regular mammograms, typically annually or biannually, depending on your age and risk factors. However, if there are still concerns about the initial mammogram findings, your physician might suggest additional imaging, such as a breast MRI, or a biopsy to obtain a definitive diagnosis.

Regarding your question about whether having your mammogram and ultrasound during your menstrual cycle could affect the results: it is possible. Hormonal changes during the menstrual cycle can lead to increased breast density and tenderness, which might affect mammogram readings. However, most radiologists are trained to interpret mammograms regardless of the menstrual cycle phase. If your breasts were particularly dense or tender during the exam, it might have contributed to the appearance of the shadow, but it is unlikely to be the sole reason for the finding.

In summary, since your ultrasound results were normal, it is a positive indication. However, it is essential to maintain open communication with your healthcare provider. Discuss your concerns and the next steps with them, as they can provide personalized advice based on your specific situation. If you feel uncertain or anxious, seeking a second opinion from another specialist in breast health can also be beneficial.

Lastly, remember that regular breast self-exams and routine screenings are crucial for early detection of any potential issues. If you notice any changes in your breasts or have new symptoms, do not hesitate to contact your healthcare provider.

Wishing you peace of mind and good health.

Best regards.
