Discomfort while sitting in the genital area?

Hello Doctor: Since last Tuesday, I have been experiencing a very sore feeling in my private area when sitting on a chair, which lasts for less than three hours.
When I stand up, the discomfort goes away.
Last week, I occasionally felt a sudden sharp pain.
I visited a gynecologist who said that there appeared to be no external issues (no skin lesions).
However, since I have not engaged in sexual intercourse, an internal examination was not possible.
As of today, sitting still feels very uncomfortable, and I can only wear loose-fitting pants; wearing tight pants causes discomfort in that area.
I started taking medication for a urinary tract infection on Monday, but as of today, I still cannot sit comfortably.
Walking and lying down are fine; the issue only occurs when sitting (and there is no discharge).
I would like to ask the doctor if there could be an issue with my urethra, or if there are other possibilities.
Thank you for your assistance.

The response suggests that it could be a fungal infection, and antifungal ointment can be applied to the affected area for relief.
Additionally, it may also be due to localized nerve compression, so please seek medical evaluation.
Sincerely, Dr.

Discomfort in the genital area while sitting can be a distressing issue, and it is essential to explore the potential causes and solutions. Based on your description, the discomfort seems to be primarily associated with prolonged sitting, leading to a sensation of soreness and occasional sharp pain. Since you have already consulted a gynecologist who found no visible external issues, we can consider several possible underlying causes.

Possible Causes
1. Nerve Compression: Sitting for extended periods can lead to compression of the nerves in the pelvic region, particularly the pudendal nerve, which innervates the genital area. This compression can cause discomfort, tingling, or sharp pain.

2. Pelvic Floor Dysfunction: The pelvic floor muscles support the pelvic organs, and any dysfunction or tightness in these muscles can lead to discomfort while sitting. This condition can be exacerbated by prolonged sitting, leading to muscle fatigue and soreness.

3. Coccyx Issues: If there is any trauma or strain to the coccyx (tailbone), it can cause pain when sitting. This pain can radiate to the surrounding areas, including the genital region.

4. Inguinal Hernia: Although less common, an inguinal hernia can cause discomfort in the groin area, especially when sitting or straining. This condition would typically require a physical examination for diagnosis.

5. Urinary Tract Issues: While you mentioned that you have been taking medication for a bladder infection, it is essential to rule out any underlying urinary tract issues, such as interstitial cystitis or bladder irritation, which can cause discomfort in the pelvic area.

6. Skin or Soft Tissue Issues: Although the gynecologist did not find any external problems, conditions such as vulvodynia or other skin irritations could still be a factor. These conditions can cause pain or discomfort in the genital area, particularly when pressure is applied.

Solutions and Recommendations
1. Posture and Seating: Evaluate your sitting posture and the type of chair you are using. A chair with good lumbar support and a cushion designed for pelvic comfort can help alleviate pressure on the genital area. Consider using a donut-shaped cushion or a wedge cushion to relieve pressure on the coccyx and pelvic floor.

2. Frequent Breaks: If your work or daily activities require prolonged sitting, try to take regular breaks. Stand up, stretch, and walk around every 30 minutes to relieve pressure and improve blood circulation.

3. Pelvic Floor Exercises: Engaging in pelvic floor exercises, such as Kegel exercises, can help strengthen the pelvic floor muscles and improve overall function. This can alleviate discomfort and prevent future issues.

4. Heat Therapy: Applying a warm compress to the pelvic area may help relax tight muscles and reduce discomfort. A warm bath can also provide relief.

5. Consult a Specialist: If the discomfort persists, consider consulting a physical therapist specializing in pelvic floor dysfunction. They can provide targeted exercises and treatments to address any underlying issues.

6. Follow-Up with a Urologist: Since you mentioned the possibility of urinary issues, a follow-up with a urologist may be beneficial. They can conduct further evaluations to rule out any urinary tract conditions.

7. Avoid Tight Clothing: As you noted that wearing tight clothing exacerbates the discomfort, opt for loose-fitting garments that do not apply pressure to the genital area.

Conclusion
While discomfort in the genital area while sitting can stem from various causes, addressing factors such as posture, seating arrangements, and pelvic floor health can significantly improve your situation. If symptoms persist or worsen, further evaluation by specialists may be necessary to ensure a comprehensive approach to your health. Remember, it is essential to listen to your body and seek help when needed.
