Consultation on bowel issues?

Hello doctor, I am 20 years old.
Recently, I visited a company that performs colonoscopies and endoscopies, which made me start paying attention to my bowel movements.
My stools are generally normal, occurring 1-2 times a day, but sometimes I notice bright red blood when wiping, and there is a slight stinging sensation around my anus.
I examined my stool closely, and some parts of it appeared to be slightly red (I am not sure if the toilet water was also slightly red; I looked closely for about 5 minutes).
Other than that, I have no other abnormal symptoms.

Previously, during my freshman year, I had a bout of gastroenteritis after eating a chicken wing that had fallen on the floor (it was raining that day...
I rinsed it with hot water).
That episode lasted about 10 days, and I lost 5 kg.
After that, I don't know if it was due to the food at school, but I experienced gastroenteritis more frequently, losing about 10 kg over the year.
I visited a general clinic, and the doctor diagnosed me with irritable bowel syndrome.
Later, my condition improved, but it seems my appetite and digestion have not returned to what they used to be (due to heavy coursework and late nights from previous camp activities).

I would like to ask the doctor if my stool situation is considered normal.
If further examination is needed, should I see a surgeon, an internist, or a gastroenterologist?

Dr. Ke Fangxu reply Surgery

This situation certainly cannot be considered normal.
I also agree with the previous physician's diagnosis of irritable bowel syndrome (IBS).
Therefore, in addition to the usual advice regarding diet and lifestyle, pharmacological treatment should also be beneficial.
If there continues to be a disruption in routine along with an abnormal diet, it is understandable that symptoms would not improve.

When it comes to bowel health, it is essential to be attentive to any changes in your bowel habits, especially when you notice symptoms such as blood in your stool or discomfort in the anal area. Based on your description, it seems that you are experiencing some concerning symptoms, including the presence of bright red blood and a stinging sensation around the anus. While you mentioned that your bowel movements are generally normal, the occasional appearance of blood and discomfort warrants further investigation.

The presence of bright red blood in your stool can indicate several conditions, ranging from benign to more serious issues. Common causes include hemorrhoids, anal fissures, diverticular disease, or even more serious conditions like inflammatory bowel disease (IBD) or colorectal cancer. Given your age and the fact that you have had a history of gastrointestinal issues, it is crucial to take these symptoms seriously.

You mentioned that you previously experienced gastroenteritis after consuming contaminated food, which could have contributed to your ongoing digestive issues. While irritable bowel syndrome (IBS) is a common diagnosis, especially among young adults, it is essential to rule out other potential causes of your symptoms. The fact that you have lost weight and experienced changes in your appetite and digestion may indicate that your gastrointestinal health needs closer examination.

In terms of next steps, I recommend scheduling an appointment with a gastroenterologist, who specializes in digestive health. They can perform a thorough evaluation, which may include a physical examination, a review of your medical history, and possibly diagnostic tests such as a colonoscopy or flexible sigmoidoscopy. These procedures allow for direct visualization of the colon and rectum, helping to identify any abnormalities that may be causing your symptoms.

Additionally, it is essential to maintain a healthy lifestyle, including a balanced diet rich in fiber, staying hydrated, and managing stress, as these factors can significantly impact your digestive health. Since you mentioned that your appetite has not been as good as before, consider consulting a nutritionist who can help you develop a meal plan that supports your digestive health and overall well-being.

In summary, while your bowel habits may seem normal at times, the presence of blood and discomfort should not be overlooked. Seeking medical advice from a gastroenterologist is the best course of action to ensure that any underlying issues are addressed promptly. Early intervention can lead to better outcomes and help you maintain a healthy digestive system moving forward.
