Menstrual irregularities
Hello doctor, I would like to ask about my menstrual cycle.
After my period came in March, I haven't had it since.
I started menstruating when I was in fifth grade, and I usually have my period about every two months.
Is this serious? Also, does staying up late and insomnia have an impact? Does obesity affect this as well? Thank you.

Dear online friend,
I am glad to receive your letter.
You would like to know if staying up late, insomnia, and obesity can affect your menstrual cycle.
Menstruation is a part of the smooth functioning of female hormones, and a regular menstrual cycle indicates that these hormones are operating effectively.
There are many reasons for missed periods, including issues with the sex hormone secretion system, lifestyle and psychological factors, medication effects, and other endocrine system problems.
Issues with the sex hormone secretion system can include conditions such as polycystic ovary syndrome, pituitary tumors, and hyperprolactinemia.
Lifestyle and psychological factors, such as smoking, staying up late, insomnia, work stress, weight changes, and excessive consumption of cold beverages, can also impact the menstrual cycle.
Medication effects may arise from anti-inflammatory drugs and antibiotics.
Other endocrine system issues, such as thyroid dysfunction, can also play a role.
Is this serious? I believe it should not be taken lightly.
I recommend maintaining a balanced diet, avoiding late nights, ensuring adequate sleep, reducing the intake of cold beverages, and cultivating a healthy lifestyle to help regulate your menstrual cycle.
Wishing you health and happiness!
Sincerely,
Yen Su-Mei

Menstrual irregularities can be a source of concern for many individuals, and understanding the underlying causes and effects is essential for managing one's health. In your case, experiencing a significant change in your menstrual cycle, such as having periods only every two months after initially starting menstruation, can be indicative of several factors that may need to be addressed.
Firstly, it is important to recognize that menstrual cycles can vary widely among individuals, especially during the early years of menstruation. It is not uncommon for young individuals to experience irregular cycles as their bodies are still adjusting to hormonal changes. However, if you have noticed a significant change in your cycle, such as a complete absence of menstruation for several months, it is advisable to consult a healthcare professional for a thorough evaluation.
Several factors can contribute to menstrual irregularities, including lifestyle choices, hormonal imbalances, and underlying medical conditions. Here are some key factors that may be influencing your situation:
1. Stress and Sleep Patterns: Chronic stress, lack of sleep, and irregular sleep patterns can significantly impact hormonal balance. The hypothalamus, which regulates the menstrual cycle, can be affected by stress and sleep deprivation, leading to irregular periods. If you are experiencing insomnia or frequently staying up late, it could be contributing to your menstrual irregularities.
2. Weight and Body Composition: Obesity or significant weight fluctuations can also affect menstrual cycles. Excess body fat can lead to increased estrogen levels, which may disrupt the normal hormonal balance necessary for regular ovulation and menstruation. Conversely, being underweight can also lead to irregular periods due to insufficient body fat to support regular hormonal function.
3. Hormonal Imbalances: Conditions such as polycystic ovary syndrome (PCOS) can cause irregular menstrual cycles. PCOS is characterized by hormonal imbalances that can lead to missed periods, weight gain, and other symptoms. If you have other symptoms such as acne, excessive hair growth, or difficulty losing weight, it may be worth discussing with your healthcare provider.
4. Thyroid Function: The thyroid gland plays a crucial role in regulating metabolism and hormonal balance. Hypothyroidism (an underactive thyroid) or hyperthyroidism (an overactive thyroid) can lead to menstrual irregularities. A simple blood test can help assess thyroid function.
5. Lifestyle Factors: Diet, exercise, and overall lifestyle can also impact menstrual health. A balanced diet and regular physical activity can help maintain a healthy weight and hormonal balance, while excessive exercise or extreme dieting can lead to irregular periods.
In summary, while occasional irregularities in menstrual cycles can be normal, persistent changes warrant further investigation. It is essential to consider the potential impact of stress, sleep patterns, weight, and hormonal health on your menstrual cycle. Consulting with a healthcare provider can help identify any underlying issues and provide guidance on how to manage your menstrual health effectively. They may recommend lifestyle changes, hormonal evaluations, or other interventions based on your specific situation. Taking proactive steps to address these factors can help restore regularity to your menstrual cycle and improve your overall well-being.
