Chronic nasal congestion with post-nasal drip?
I have been experiencing intermittent post-nasal drip for three years, with the most severe symptoms occurring in the morning upon waking, which improve by midday.
The mucus is clear.
I do not take antihistamines; I have tried herbal medicine, which was effective when taken, but the symptoms return when I stop.
I have noticed that drinking hot water and taking hot baths significantly improve the post-nasal drip, but exercise does not seem to help.
I have had chronic nasal congestion for 5 to 6 years, which does not affect my breathing.
I have also experienced tinnitus for 6 years, but my sleep quality is good, and the tinnitus symptoms disappear during that time.
I have undergone two hearing tests, both of which were normal, with the last one being last year.
I mentioned that the tinnitus is more severe in my left ear, which also experiences sharp pain, while the right ear is less affected.
An ear specialist performed an endoscopic examination, which showed no abnormalities or inflammation.
Could the lack of inflammation in my ears be related to nasal congestion or post-nasal drip? Is there a connection between untreated chronic nasal congestion and post-nasal drip? What could be the underlying causes of the post-nasal drip? Is it related to sleep and stress? My symptoms are worse in the morning, and I drink hot water and do some light stretching before getting out of bed, ensuring I keep warm, but this does not improve the post-nasal drip symptoms.

Hi Niza: If your ears are itchy and there is no obvious inflammation upon examination, it is commonly indicative of chronic otitis externa.
It is advisable to reduce the use of cotton swabs for ear cleaning.
If you are experiencing post-nasal drip with clear nasal discharge, it is most commonly associated with chronic rhinitis.
You can use medications to control and reduce secretions; if you prefer not to take medication, you can purchase a nasal irrigation device to rinse your nose with saline solution, which can also provide some improvement.
Continuing to exercise is recommended, as it can help alleviate nasal congestion.
Wishing you peace, health, and well-being.
Sincerely, Dr.

More Info
Chronic nasal congestion and post-nasal drip can be quite bothersome and may significantly affect your quality of life. Based on your description, it seems you have been experiencing these symptoms for several years, with a pattern of worsening in the mornings and some improvement throughout the day. Let's break down the potential causes and management strategies for your condition.
Understanding the Symptoms
1. Chronic Nasal Congestion: This can arise from various factors, including allergies, structural abnormalities (like a deviated septum), or chronic rhinosinusitis. The fact that your nasal discharge is clear suggests that it might be related to allergies or non-infectious rhinitis rather than a bacterial infection.
2. Post-Nasal Drip: This occurs when excess mucus accumulates in the back of the throat, often leading to a sensation of mucus dripping down. This can be caused by chronic sinusitis, allergies, or even irritants in the environment. The improvement you experience after hot showers could indicate that steam helps to thin the mucus, making it easier to manage.
3. Ear Symptoms: The ear symptoms you describe, such as tinnitus (ear ringing) and pain, can indeed be related to nasal congestion. The Eustachian tube, which connects the middle ear to the back of the nose, can become blocked due to nasal congestion, leading to pressure changes and discomfort in the ear. This is often exacerbated by allergies or sinus issues.
Potential Causes
- Allergic Rhinitis: If you have not been taking antihistamines, it might be worth considering allergy testing. Allergies can lead to chronic inflammation in the nasal passages, resulting in congestion and post-nasal drip.
- Chronic Sinusitis: Even if you do not have a current infection, chronic inflammation of the sinuses can lead to ongoing symptoms. This can be evaluated through imaging studies like a CT scan if necessary.
- Environmental Irritants: Exposure to smoke, pollution, or strong odors can exacerbate nasal symptoms. Identifying and minimizing exposure to these irritants can help.
- Gastroesophageal Reflux Disease (GERD): Sometimes, acid reflux can cause post-nasal drip symptoms. If you notice that your symptoms worsen after eating or when lying down, this might be a factor.
Management Strategies
1. Hydration: Staying well-hydrated can help thin mucus. Drinking warm fluids, as you mentioned, can provide relief.
2. Steam Inhalation: Regularly inhaling steam can help to moisten and clear nasal passages. You can do this through hot showers or using a humidifier.
3. Nasal Irrigation: Using saline nasal sprays or a neti pot can help clear mucus and allergens from the nasal passages. This can be particularly effective in managing post-nasal drip.
4. Allergy Management: If allergies are suspected, consider discussing with your healthcare provider about starting antihistamines or nasal corticosteroids. These can help reduce inflammation and mucus production.
5. Avoiding Irritants: Try to identify and avoid any known triggers, such as smoke, strong perfumes, or other allergens.
6. Consultation with Specialists: If symptoms persist, it may be beneficial to consult an ENT specialist. They can perform a thorough examination and possibly recommend further testing or treatments, such as allergy testing or imaging studies.
Conclusion
Chronic nasal congestion and post-nasal drip can be complex issues often intertwined with allergies, sinus health, and environmental factors. While your symptoms may not significantly impact your breathing or sleep quality, addressing them can enhance your overall well-being. It’s essential to work closely with healthcare professionals to identify the underlying causes and develop an effective management plan tailored to your needs.
