Long-Term Use of Silymarin for Liver Health: Benefits and Risks - Gastroenterology and Hepatology

Liver protection supplements issues?

(1) I previously had abnormal liver function (GPT 70), and my doctor prescribed Kojar (Silymarin) capsules.
After three months, my liver function returned to normal, and the medication was discontinued.
Over the past few years, my liver enzyme levels have fluctuated (GPT 35-60).
Can I take Silymarin long-term (150-300 mg daily) to help with mild liver inflammation and to prevent liver cirrhosis? I did not experience any side effects during the previous treatment, but I am concerned about any potential long-term effects on my liver or overall health.
(2) If I take this medication, will it cause any confusion in blood test results that could delay the treatment timing for hepatitis B? It seems that doctors often suggest checking viral load when enzyme levels rise.
Thank you, doctor.

Hello Mr.
Lin: 1.
If there is a clinical need for Silymarin, it can be taken long-term without any reported significant side effects, but its effectiveness is not guaranteed.
According to health insurance regulations, treatment must be discontinued if there is no improvement.
2.
Hepatitis B virus is not affected by Silymarin and can only be treated with antiviral medications.

Long-term use of Silymarin, commonly derived from the Milk Thistle plant, has been a topic of interest in the medical community, particularly concerning its effects on liver health. Silymarin is often recommended for individuals with liver dysfunction, as it is believed to have hepatoprotective properties. However, the decision to use it long-term should be approached with caution and under medical supervision.

1. Long-term Use and Safety: Silymarin is generally considered safe for long-term use, especially in individuals with mild liver inflammation or elevated liver enzymes. Studies have shown that Silymarin can help improve liver function and may provide some protective effects against liver damage. However, it is essential to note that while many patients report benefits, the scientific evidence supporting its efficacy is mixed. Some studies suggest that Silymarin can help reduce liver inflammation and fibrosis, but it is not a substitute for conventional treatments for liver diseases.
The typical dosage ranges from 150 mg to 300 mg per day, as you mentioned. While many individuals tolerate Silymarin well, potential side effects can include gastrointestinal disturbances such as diarrhea, nausea, and abdominal pain. Rarely, allergic reactions may occur, which could manifest as skin rashes or itching. If you experience any unusual symptoms, it is advisable to consult your healthcare provider.

Regarding your concern about long-term use affecting liver health, current evidence does not indicate that Silymarin poses significant risks when taken as directed. However, it is crucial to have regular follow-ups with your healthcare provider to monitor liver function and ensure that any underlying conditions are appropriately managed.

2. Impact on Blood Test Results and Hepatitis B Treatment: Silymarin is not known to interfere with blood tests for liver function or hepatitis B viral load. The liver enzymes (such as GPT) can fluctuate due to various factors, including diet, alcohol consumption, and underlying liver conditions. If your healthcare provider suspects hepatitis B or any other liver disease, they will likely recommend testing regardless of Silymarin use.
It is important to communicate openly with your healthcare provider about all supplements and medications you are taking. This transparency allows them to make informed decisions about your treatment plan and avoid any potential delays in necessary interventions.

In summary, while Silymarin can be a beneficial adjunct in managing liver health, it should not replace standard medical care for liver diseases. Regular monitoring and communication with your healthcare provider are essential to ensure that your liver health is adequately managed and that any potential issues are addressed promptly. Always consult your healthcare provider before making any changes to your medication or supplement regimen, especially for long-term use.
