Right lower abdominal pain?
Hello Doctor: For the past three months, I have been experiencing intermittent abdominal pain, particularly in the lower right abdomen, just slightly below the navel.
It feels like something is stuck there, causing a bloated and painful sensation.
During these episodes, my lower back feels a bit sore, and sometimes the pain spreads throughout my entire abdomen.
My stools have been soft, resembling undigested food, and occasionally I experience diarrhea.
When the pain occurs, my breath feels a bit off, and I have a reduced appetite.
I previously went to the hospital for an ultrasound and a stool occult blood test, and the doctor said everything was fine.
This issue has been bothering me for two to three months, and I would like to ask the doctor what these symptoms might indicate.
Mr. He, 20~29 year old female. Ask Date: 2015/09/15
Dr. Chen Qifeng reply Gastroenterology and Hepatology
Hello, it may be gastrointestinal dysfunction secondary to gastroenteritis.
It is recommended to avoid oily, dairy, and irritating foods, and to follow a bland diet.
If there is no improvement, you may consult a gastroenterologist at the Taoyuan Hospital, Ministry of Health and Welfare.
Sincerely, Dr.
Chi-Feng Chen, Gastroenterology.

Right lower abdominal pain can be a complex issue with various potential causes, especially when accompanied by symptoms such as changes in bowel habits, discomfort in the lower back, and alterations in appetite. Given your description of the pain being localized to the right lower abdomen, along with sensations of bloating and soft stools, it is essential to consider several possible underlying conditions.
One common cause of right lower abdominal pain is appendicitis, which is an inflammation of the appendix. This condition typically presents with sharp pain that starts around the navel and then moves to the right lower quadrant. However, since you mentioned that previous imaging and tests did not indicate any issues, this may be less likely.
Another possibility is gastrointestinal issues such as irritable bowel syndrome (IBS) or inflammatory bowel disease (IBD), which can cause abdominal pain, changes in bowel habits, and bloating. IBS is characterized by recurrent abdominal pain associated with changes in bowel habits, including diarrhea or constipation. If your stools are often soft or you experience diarrhea, IBS could be a consideration.
Additionally, conditions such as diverticulitis, which involves inflammation or infection of small pouches that can form in the walls of the intestines, could also lead to similar symptoms. This condition typically presents with localized pain, often in the lower left abdomen, but can sometimes cause discomfort in the right lower quadrant as well.
Since you mentioned that you have experienced a decrease in appetite and some gastrointestinal distress, it may also be worth considering the role of anxiety and stress in your symptoms. Psychological factors can significantly impact gastrointestinal function, leading to symptoms such as abdominal pain and changes in bowel habits. Anxiety can exacerbate gastrointestinal issues, leading to a cycle of discomfort and stress.
Given that you have already undergone ultrasound and fecal occult blood tests, which returned normal results, it may be beneficial to follow up with your healthcare provider for further evaluation. They may recommend additional tests, such as a CT scan, to rule out other potential causes, including kidney stones or gynecological issues if you are female.
In the meantime, keeping a symptom diary could help identify any patterns or triggers related to your pain. Note when the pain occurs, its intensity, associated symptoms, and any dietary factors that may contribute to your discomfort. This information can be valuable for your healthcare provider in determining the next steps for diagnosis and treatment.
In summary, while right lower abdominal pain can arise from various causes, including gastrointestinal disorders, psychological factors, and potential anatomical issues, a thorough evaluation by a healthcare professional is crucial for accurate diagnosis and effective management. If your symptoms persist or worsen, do not hesitate to seek medical attention for further investigation.
