Issues with the big toe?

Hello Dr.
Wu, I would like to ask you about an issue I'm experiencing.
Whenever I wear shoes, regardless of the type or size, if I stand for 30 minutes or walk for 30 minutes, both of my big toes turn purple and are painful for 3 to 5 days before improving.
Soaking them in hot water provides some relief, but they still hurt a lot afterward.
It has been painful for 2 days now (I soaked them in hot water at night), and there hasn't been much improvement.
I don't feel pain when pressing on the nails, but there is a sharp pain when I press on the area.
This pain often occurs because of wearing cloth shoes.
Sometimes I can't go out without shoes, and I really hope to resolve this issue quickly.
When I was younger, I had an incident where my toenail was lifted due to a door, and since then, I've had this problem whenever I wear shoes.
I'm not sure if this is the reason.

Māomī, 20~29 year old female. Ask Date: 2015/09/16

Dr. Wu Yuxin reply Dermatology

It could be paronychia caused by an ingrown toenail, but based on the description alone and without seeing the lesion, it cannot be confirmed.
It is advisable to have a physician examine it for a proper diagnosis.

Hello! Thank you for reaching out with your concerns regarding your big toe pain. Based on your description, it sounds like you may be experiencing a condition known as "hallux valgus," commonly referred to as a bunion, or possibly a form of metatarsalgia, which is pain in the ball of the foot. Given your history of trauma to the toenail and the ongoing pain after wearing shoes, there are several potential causes and solutions to consider.

Possible Causes of Big Toe Pain
1. Bunion (Hallux Valgus): This is a common condition where the big toe deviates towards the second toe, causing a bony bump on the side of the foot. This can lead to pain, especially when wearing shoes that do not accommodate the deformity.

2. Nail Trauma: The history of your toenail being injured could have led to changes in how the nail grows, potentially causing ingrown toenails or other nail-related issues that can contribute to pain.

3. Metatarsalgia: This condition involves pain and inflammation in the ball of the foot, which can be exacerbated by certain types of footwear or prolonged standing and walking.

4. Neuroma: A neuroma, particularly a Morton's neuroma, can cause pain in the toes and is often related to irritation of the nerves between the toes, which can be aggravated by tight or ill-fitting shoes.

5. Arthritis: Conditions like osteoarthritis or rheumatoid arthritis can also lead to joint pain in the big toe, especially if there is a history of joint issues in your family.

Recommended Solutions for Relief
1. Footwear: It’s crucial to wear shoes that provide ample space for your toes. Look for shoes with a wide toe box and avoid high heels or narrow shoes that can exacerbate the pain. Consider using orthotic inserts that provide additional support and cushioning.

2. Ice Therapy: Applying ice to the affected area can help reduce inflammation and numb the pain. Try icing your toes for 15-20 minutes several times a day, especially after activities that aggravate the pain.

3. Warm Water Soaks: As you mentioned, soaking your feet in warm water can provide temporary relief. You might also consider adding Epsom salts to the water, which can help reduce swelling and soothe sore muscles.

4. Pain Relief Medications: Over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen can help reduce pain and inflammation. However, consult with a healthcare professional before starting any medication, especially if you have other health conditions.

5. Physical Therapy: A physical therapist can provide exercises to strengthen the muscles in your feet and improve flexibility, which may help alleviate some of the pain.

6. Consult a Specialist: If the pain persists, it may be beneficial to see a podiatrist or orthopedic specialist. They can perform a thorough examination, possibly including imaging studies like X-rays, to determine the exact cause of your pain and recommend appropriate treatments, which may include custom orthotics or, in severe cases, surgical options.

7. Avoiding Aggravating Activities: Until you have a clearer understanding of the cause of your pain, it may be wise to limit activities that exacerbate your symptoms, such as prolonged standing or walking in shoes that cause discomfort.

Conclusion
It's understandable to feel frustrated with ongoing pain, especially when it impacts your daily activities. By taking proactive steps to address your footwear, using home remedies, and seeking professional advice, you can work towards finding relief. Remember, early intervention can often prevent more serious issues down the line, so don’t hesitate to seek further evaluation if your symptoms do not improve. Wishing you a speedy recovery!
