Intermenstrual bleeding with brownish discharge?

Doctor, I'm sorry to bother you.
My period started on August 28, and from September 15 to 17, I noticed brown discharge during urination.
Today, I also had a soft discharge that resembled endometrial shedding.
I visited a gynecologist in the evening; the doctor did not perform an internal examination but conducted a pelvic ultrasound.
They did not check my ovaries and only mentioned that it looked like there was bleeding from a scar from a previous cesarean section.
However, it's been three years since I gave birth; could the scar still be healing? Six years ago, I had cervical erosion during early pregnancy, but I had a miscarriage at 15 weeks.
It took three years to conceive again, but due to blood pressure issues, the baby did not grow properly in the uterus and had insufficient activity, leading to an early cesarean delivery.
Since then, I have experienced abdominal discomfort and pain during ovulation, feeling like I need to pass gas but can't.
I still want to have a second child, but I'm worried about my health issues.
What should I do? What tests should I undergo?

1.
There are many causes of intermenstrual bleeding, including irregular menstruation, inflammation, polyps, tumors, pregnancy, and more.
A further examination is necessary to determine the specific cause.

2.
Ovulation can induce uterine contractions, leading to pressure on surrounding organs and causing pain.
Conditions such as endometriosis, pelvic adhesions, and chronic inflammation should be considered.
Please seek medical evaluation.

Respectfully, Dr.

Brown discharge can be a concerning symptom for many women, especially when it occurs outside of the normal menstrual cycle. Understanding what it means for your reproductive health is crucial, particularly given your medical history and current symptoms.

Firstly, brown discharge typically indicates old blood that is being expelled from the body. This can happen for several reasons, including the end of a menstrual period, ovulation, or even early pregnancy. In your case, since you mentioned that your last menstrual period started on August 28 and you experienced brown discharge between September 15 and 17, it could be related to ovulation or the tail end of your menstrual cycle. However, given your history of cervical issues and previous pregnancies, it’s essential to consider other factors.

You mentioned that you had a cesarean section three years ago and that the doctor noted a scar from the surgery. While it is uncommon for a cesarean scar to cause ongoing bleeding years later, it is not impossible. Scar tissue can sometimes lead to complications, including abnormal bleeding or pain. The fact that you have experienced discomfort during ovulation may suggest that there could be some underlying issues related to your reproductive organs, such as adhesions or endometriosis, which can cause pain and irregular bleeding.

Your history of cervical erosion and miscarriage also raises some concerns. Cervical erosion can lead to abnormal discharge and bleeding, especially if there are changes in the cervical tissue. It’s important to monitor these symptoms closely, as they can sometimes indicate underlying conditions that may affect your fertility.

Given your desire to conceive again, it would be prudent to undergo a comprehensive evaluation. Here are some recommended steps:
1. Pelvic Ultrasound: Since your doctor performed a pelvic ultrasound, it would be beneficial to ensure that both your uterus and ovaries are healthy. This imaging can help identify any abnormalities, such as fibroids or cysts.

2. Hysteroscopy: If there are concerns about the uterine lining or any intrauterine abnormalities, a hysteroscopy may be warranted. This procedure allows for direct visualization of the inside of the uterus and can help identify any issues that may affect implantation or pregnancy.

3. Hormonal Evaluation: Hormonal imbalances can lead to irregular cycles and may affect fertility. A blood test to check hormone levels, including progesterone, estrogen, and thyroid hormones, can provide insight into your reproductive health.

4. Cervical Examination: Given your history of cervical issues, a thorough examination of the cervix, including a Pap smear, may be necessary to rule out any abnormalities or infections that could contribute to your symptoms.

5. Fertility Assessment: If you are actively trying to conceive, consider consulting with a fertility specialist. They can provide a more detailed assessment of both partners’ reproductive health and recommend appropriate interventions if needed.

6. Lifestyle Modifications: Maintaining a healthy lifestyle, including a balanced diet, regular exercise, and stress management, can improve overall reproductive health and increase the chances of conception.

In conclusion, while brown discharge can be a normal occurrence, your specific history and symptoms warrant further investigation. It’s essential to communicate openly with your healthcare provider about your concerns and to pursue the necessary evaluations to ensure your reproductive health is in optimal condition for future pregnancies. Remember, seeking help early can make a significant difference in your journey toward expanding your family.
