Unilateral eyelid swelling?

Hello, doctor.
I would like to inquire about my eyelid.
A year ago, my left upper eyelid suddenly swelled up one day while I was wearing monthly disposable contact lenses.
I visited 4 to 5 clinics but did not receive a complete answer.
Since then, I have stopped wearing contact lenses because I thought I might be allergic to them.
However, it has been a year, and I have not worn contact lenses since.
My left upper eyelid is still swollen; it has neither disappeared nor become larger.
I would like to ask the doctor if there is still hope for my left upper eyelid.
Only the left side is affected; the right side is normal.

Brian, 20~29 year old female. Ask Date: 2015/09/23

Dr. Gao Pilin reply Ophthalmology

Hello Brian: If the swelling is due to a single lump inside, it could be a chalazion.
If the entire area is swollen, it might be edema.
I'm not sure which type of swelling you are referring to.
If it's an allergic reaction, the swelling should decrease after medication, so I recommend that you get a reevaluation from an ophthalmologist.
In Taiwan, the ophthalmology department at Puzi Hospital is open Monday to Friday from 8:00 AM to 12:00 PM and Wednesday afternoons from 1:00 PM to 4:30 PM.
I hope we have the opportunity to assist you.
Best regards, Gao Pilin.

More Info

Persistent swelling of one eyelid can be a concerning issue, especially when it has lasted for an extended period, such as a year in your case. There are several potential causes for this condition, and understanding them can help guide you toward the appropriate treatment.

Possible Causes of Eyelid Swelling
1. Allergic Reactions: Allergies to substances such as pollen, dust, pet dander, or even cosmetics can cause localized swelling. Since you mentioned that you were wearing contact lenses when the swelling began, it is possible that you had an allergic reaction to the lens material or the solution used for cleaning them.

2. Chalazion or Stye: A chalazion is a blocked oil gland in the eyelid, which can lead to swelling. A stye, on the other hand, is an infection of the oil glands and can also cause localized swelling and tenderness. Both conditions can cause persistent swelling if not treated properly.

3. Infection: Bacterial or viral infections can lead to swelling of the eyelid. Conditions such as conjunctivitis (pink eye) can cause swelling, redness, and discomfort.

4. Dermatitis: Contact dermatitis, which can occur from exposure to irritants or allergens, may cause swelling and inflammation of the eyelid. This could be due to products applied near the eyes or even from the materials in your contact lenses.

5. Other Medical Conditions: Conditions such as thyroid disease (Graves' disease), which can cause swelling around the eyes, or other systemic conditions may also be responsible for persistent eyelid swelling.

Solutions and Recommendations
1. Consult an Eye Specialist: Since you have already visited multiple clinics without a satisfactory diagnosis, it may be beneficial to see a specialist, such as an ophthalmologist. They can perform a thorough examination, including imaging studies if necessary, to determine the underlying cause of the swelling.

2. Avoid Contact Lenses: As you have already stopped wearing contact lenses, continue to avoid them until you have a clear diagnosis. This will help prevent any potential allergic reactions or irritations from worsening.

3. Cold Compresses: Applying a cold compress to the swollen eyelid can help reduce inflammation and provide temporary relief. This can be done several times a day for about 10-15 minutes.

4. Topical Treatments: Depending on the diagnosis, your doctor may prescribe topical treatments, such as antihistamines for allergic reactions or antibiotic ointments for infections.

5. Oral Medications: If the swelling is due to an allergic reaction, oral antihistamines may be recommended. In cases of infection, oral antibiotics may be necessary.

6. Monitor Symptoms: Keep track of any changes in the swelling, such as increased redness, pain, or discharge, and report these to your doctor. This information can help in diagnosing the issue.

7. Consider Allergy Testing: If allergies are suspected, your doctor may recommend allergy testing to identify specific triggers that could be causing the swelling.

Conclusion
In summary, while persistent swelling of one eyelid can be frustrating and concerning, there are various potential causes, ranging from allergies to infections. It is crucial to seek a thorough evaluation from an eye specialist who can provide a definitive diagnosis and appropriate treatment plan. With the right approach, there is a good chance that your eyelid condition can be managed effectively. Remember to avoid any potential irritants and monitor your symptoms closely as you seek further medical advice.
