Left lower abdominal distension?
Hello Doctor: I have been experiencing bloating in my lower left abdomen for the past couple of months.
Initially, I consulted a gynecologist and underwent an ultrasound and a Pap smear.
Aside from a small fibroid in the uterus, there were no abnormalities.
Additionally, due to the bloating issue after dinner, I also saw a gastroenterologist, who performed an abdominal ultrasound, urine, and stool tests, all of which were normal.
Currently, the gastroenterologist has prescribed medication to see if my condition improves.
I also had an X-ray, but I need to return in two weeks to review the results.
Recently, I have noticed some pain when pressing on the left side of my pelvis.
Is this a concern? I am unsure whether this is a gynecological issue, a gastrointestinal issue, or an orthopedic issue.

More Info
Understanding left lower abdominal bloating and discomfort can be quite complex, as it may stem from various underlying causes. Based on your description, it seems you have already taken significant steps by consulting both gynecological and gastrointestinal specialists, which is commendable. The presence of a small uterine fibroid, while generally benign, can sometimes contribute to discomfort, especially if it is located in a position that exerts pressure on surrounding structures.
Potential Causes of Left Lower Abdominal Bloating
1. Gastrointestinal Issues:
- Irritable Bowel Syndrome (IBS): This functional gastrointestinal disorder can lead to bloating, gas, and discomfort, particularly after meals. Stress and dietary choices often exacerbate symptoms.
- Diverticulitis: Inflammation of diverticula (small pouches that can form in the colon) can cause localized pain and bloating, particularly in the left lower abdomen.
- Constipation: Infrequent bowel movements can lead to bloating and discomfort. The left side of the colon is often where stool accumulates, leading to increased pressure and discomfort.
- Food Intolerances: Lactose intolerance or gluten sensitivity can lead to bloating and discomfort after eating certain foods.
2. Gynecological Issues:
- Uterine Fibroids: While small fibroids are often asymptomatic, they can sometimes cause pressure symptoms or discomfort, particularly if they are located near the pelvic wall.
- Ovarian Cysts: These fluid-filled sacs can cause pain and bloating, especially if they rupture or become large.
- Endometriosis: This condition, where tissue similar to the lining inside the uterus grows outside of it, can cause significant pain and bloating, particularly during menstruation.
3. Musculoskeletal Issues:
- Muscle Strain: Activities that involve heavy lifting or awkward movements can lead to muscle strain in the abdominal wall, causing localized pain that may be mistaken for internal issues.
- Hernias: Although you mentioned not having a bulge, certain types of hernias can present with pain without an obvious external sign.
Diagnostic Considerations
Given that you have undergone various imaging studies and tests, it is crucial to follow up on the results of your X-ray and any other pending evaluations. If your symptoms persist or worsen, further investigations may be warranted, such as:
- CT Scan or MRI: These imaging modalities can provide a more detailed view of the abdominal and pelvic organs, helping to identify any abnormalities that might not be visible on ultrasound.
- Colonoscopy: If gastrointestinal issues are suspected, this procedure can help visualize the inner lining of the colon and rectum, allowing for the detection of polyps, inflammation, or other abnormalities.
Management Strategies
1. Dietary Modifications: Keeping a food diary can help identify potential triggers for your bloating. Consider reducing high-FODMAP foods, which are known to cause gas and bloating in sensitive individuals.
2. Stress Management: Since stress can exacerbate gastrointestinal symptoms, incorporating relaxation techniques such as yoga, meditation, or deep-breathing exercises may be beneficial.
3. Follow-Up Care: Regular follow-ups with your healthcare providers are essential. If symptoms persist, consider seeking a second opinion or consulting a specialist in functional gastrointestinal disorders.
Conclusion
In summary, left lower abdominal bloating can arise from a variety of causes, including gastrointestinal, gynecological, and musculoskeletal issues. It is essential to continue working closely with your healthcare providers to determine the underlying cause of your symptoms and to develop an appropriate management plan. If you experience any new or worsening symptoms, such as severe pain, fever, or changes in bowel habits, seek medical attention promptly. Your health and comfort are paramount, and addressing these concerns with your healthcare team will help ensure a comprehensive approach to your well-being.
