Gynecomastia in males?

Hello doctor, I am an 18-year-old male in my senior year of high school.
I have had a problem with enlarged breasts since the fifth grade, which began around the same time I started having an imbalanced diet and rapidly gaining weight.
During middle school, I often ate fried foods like chicken cutlets, and by my first year of high school, my weight was nearly 100 kg.
I have been continuously trying to lose weight, and my current height is 171 cm with a weight of 83 kg, which is a significant reduction.
However, the issue of enlarged breasts still persists.
Some classmates weigh over 90 kg but do not have the same problem with enlarged breasts.
While my breasts are not excessively large, they are noticeably prominent when I wear a T-shirt.
Should I continue to lose more weight, or could this be related to hormonal factors? This has been a long-standing concern for me, and I would appreciate your guidance.
Thank you, doctor.

Dr. Zhou Jiazhen reply Breast and Thyroid

Dear A: Adipose tissue contains female hormones, which can lead to gynecomastia in obese males.
Some cases are related to endocrine issues, and weight loss may not always improve the condition.
It is recommended to engage in regular exercise, maintain a normal lifestyle (avoid staying up late), and follow a light diet, then reassess the situation for improvement.

Gynecomastia, commonly referred to as enlarged male breasts, is a condition characterized by the enlargement of breast tissue in males. It is important to understand that not all men with larger chests have gynecomastia; the diagnosis is typically made when the breast tissue beneath the nipple exceeds 2 centimeters. This condition can be influenced by various factors, including hormonal imbalances, weight fluctuations, and lifestyle choices.

In your case, at 18 years old and having experienced significant weight gain during your childhood, it is possible that your enlarged breast tissue is related to both hormonal changes and body composition. During puberty, many males experience fluctuations in hormone levels, particularly an increase in estrogen relative to testosterone, which can lead to temporary gynecomastia. This condition often resolves on its own within a few years. However, if the breast tissue remains enlarged beyond this period, it may be indicative of persistent gynecomastia.

Your history of rapid weight gain and a diet high in fried foods could also contribute to the appearance of enlarged breasts. Excess body fat can lead to increased estrogen production, as adipose (fat) tissue can convert androgens into estrogens. Therefore, maintaining a healthy weight through balanced nutrition and regular exercise is crucial. While you have made progress in your weight loss journey, it may be beneficial to continue focusing on a well-rounded diet and consistent physical activity to further reduce body fat.

It is also essential to consider the role of genetics and individual body composition. Some individuals may have a predisposition to develop gynecomastia regardless of their weight. This means that even if you achieve a healthy weight, you may still have some degree of breast tissue enlargement due to genetic factors.

If you are concerned about the persistent enlargement of your breasts, it would be wise to consult with a healthcare professional, such as an endocrinologist or a plastic surgeon specializing in gynecomastia. They can evaluate your hormone levels and overall health to determine if there is an underlying condition that needs to be addressed. In some cases, gynecomastia can be treated with medications that address hormonal imbalances, or surgical options may be considered if the condition is severe and causes psychological distress.

In summary, while weight loss and a healthy lifestyle can significantly impact the appearance of gynecomastia, it is essential to recognize that hormonal factors and genetics also play a crucial role. If the condition persists despite your efforts, seeking medical advice is the best course of action to explore potential treatments and ensure your overall well-being.
