Black spots on the foreskin?
Hello Doctor, two weeks ago while I was sleeping, I noticed a slight stinging and redness on my foreskin.
I then used a cream that I had previously received from a doctor (it was prescribed for itching of the foreskin, the cream is called Bowlin Scheree Cream).
The next day, the pain was gone.
However, a few days later while showering, I discovered some elongated black spots on my foreskin, and upon closer inspection, I noticed that there seemed to be more small black spots nearby.
What could this be?

It may be changes in the skin caused by steroids.
However, it is still necessary to consult a urologist to confirm the diagnosis.

More Info
The appearance of black spots on the foreskin can be concerning and may indicate a variety of underlying conditions. Based on your description, it seems that you initially experienced some irritation and redness, which improved with the use of a topical cream. However, the subsequent development of black spots warrants further investigation.
There are several potential causes for the black spots you are observing on the foreskin:
1. Hyperpigmentation: This is a common condition where certain areas of the skin become darker than the surrounding skin. It can occur due to friction, inflammation, or irritation, which may have been triggered by the initial redness and discomfort you experienced.
2. Fungal or Bacterial Infections: Infections can lead to changes in skin color. Fungal infections, such as candidiasis, can cause irritation and may lead to darkening of the skin. Bacterial infections can also cause localized inflammation and discoloration.
3. Dermatitis: Conditions like contact dermatitis can result from an allergic reaction to products used in the genital area, including soaps, lotions, or even the cream you applied. This can lead to inflammation and subsequent hyperpigmentation.
4. Pearly Penile Papules: These are small, harmless bumps that can appear on the penis and are often mistaken for other conditions. They are usually flesh-colored or slightly darker and are not a cause for concern.
5. Moles or Skin Tags: The appearance of new moles or skin tags can also occur in this area. While most are benign, any changes in size, shape, or color should be evaluated by a healthcare professional.
6. Sexually Transmitted Infections (STIs): Certain STIs can cause changes in skin appearance, including the development of spots or lesions. If there is any chance of exposure, it is important to get tested.
Given the variety of potential causes, it is crucial to consult a healthcare professional, preferably a dermatologist or urologist, for an accurate diagnosis. They may perform a physical examination and, if necessary, conduct tests such as a skin scraping or biopsy to determine the exact nature of the spots.
In terms of treatment, it will depend on the underlying cause. If it is a fungal or bacterial infection, appropriate antifungal or antibiotic medications may be prescribed. For hyperpigmentation, treatments may include topical agents to lighten the skin or other dermatological procedures.
In the meantime, here are some general care tips:
- Keep the Area Clean and Dry: Maintain good hygiene to prevent infections. Gently wash the area with mild soap and water, and ensure it is thoroughly dried afterward.
- Avoid Irritants: Be cautious with products you apply to the area. If you suspect a reaction to a cream or soap, discontinue use and consult your doctor.
- Wear Loose-fitting Underwear: This can help reduce friction and irritation in the genital area.
- Monitor Changes: Keep an eye on the spots for any changes in size, shape, or color, and report these to your healthcare provider.
In conclusion, while the appearance of black spots on the foreskin can be alarming, many potential causes are treatable. Seeking medical advice is essential to ensure proper diagnosis and management.
