Spinal issues?

Hello Dr.
Wang: My father has worn out the cartilage in his lumbar vertebrae L2, L3, L4, and L5.
The intervertebral discs are protruding and pressing on the nerves, causing bone spurs.
He is currently experiencing severe back pain and has difficulty standing, and the muscle in his thigh is continuously atrophying.
Could you please advise which type of hospital he should visit, orthopedic or neurosurgery? What are the differences between the two specialties? Thank you! My father lives in Taipei; could you recommend some reputable spine specialists in Taipei? I appreciate your help!

Xing Er, 40~49 year old female. Ask Date: 2015/10/05

Dr. Wang Shouji reply Orthopedics

Hello: In cases of degenerative arthritis of the spine with associated conditions, if conservative treatment (medication and rehabilitation) is ineffective, and MRI shows spinal compression, surgical decompression is necessary to restore neurological function and reduce pain.
In fact, both orthopedic and neurosurgical specialists can perform the surgery, provided they are specialized in spinal surgery.
Thank you!

When dealing with severe spine issues, such as those described in your father's case, it is crucial to choose the right specialist to ensure proper diagnosis and treatment. In Taipei, you have the option to consult either an orthopedic surgeon or a neurosurgeon, as both specialties can address spinal problems, but they do so from slightly different perspectives.

Orthopedic surgeons focus on the musculoskeletal system, which includes bones, joints, ligaments, tendons, and muscles. They are trained to handle a variety of conditions related to the spine, including degenerative disc disease, herniated discs, and spinal stenosis. In cases like your father's, where there is significant degeneration of the vertebrae and associated pain, an orthopedic surgeon can provide treatments ranging from conservative management (like physical therapy and pain management) to surgical interventions (like spinal fusion or decompression surgery).

On the other hand, neurosurgeons specialize in the nervous system, which includes the brain, spinal cord, and peripheral nerves. They are well-equipped to handle conditions that involve nerve compression, such as those caused by herniated discs or bone spurs pressing on spinal nerves. If your father's condition is causing significant nerve-related symptoms, such as pain radiating down the legs or muscle weakness, a neurosurgeon might be the more appropriate choice.

In Taipei, there are several reputable hospitals and specialists you can consider:
1. National Taiwan University Hospital (NTUH): This is one of the leading medical centers in Taiwan, with a strong orthopedic and neurosurgery department. You can consult Dr. Wang Cheng-Yu, a well-known orthopedic surgeon, or Dr. Hsu Chih-Hsiang, a neurosurgeon specializing in spinal disorders.

2. Taipei Veterans General Hospital: This hospital has a comprehensive spine center and offers both orthopedic and neurosurgical services. Dr. Lee Wei-Chung is a respected orthopedic surgeon there, while Dr. Tsai Shih-Hsiang is noted for his expertise in neurosurgery.

3. Chang Gung Memorial Hospital: Another top-tier hospital in Taipei, it has a dedicated spine surgery department. Dr. Wu Chih-Hsiang is an orthopedic surgeon who specializes in spinal conditions, and Dr. Chen Chih-Hsiang is a neurosurgeon with extensive experience in treating complex spine issues.

4. Shin Kong Wu Ho-Su Memorial Hospital: This hospital also has a good reputation for spine surgery. You might consider consulting with Dr. Chen Yi-Cheng, who has a focus on minimally invasive spine surgery.

Before making an appointment, it is advisable to gather all relevant medical records, including imaging studies (like MRIs or CT scans) and any previous treatment records. This information will help the specialist understand the severity of your father's condition and tailor a treatment plan accordingly.

In summary, both orthopedic and neurosurgical specialists can effectively treat severe spine issues, but the choice may depend on the specific symptoms and the underlying cause of the problem. Consulting with either type of specialist in a reputable hospital in Taipei will provide your father with the best chance for relief from his pain and improvement in his quality of life.
