Irregular menstruation?

Hello Doctor, I previously asked a question, and this time my menstrual flow is lighter than before and is delayed by seven days.
I usually experience breast tenderness, but this time I don't, although I do have premenstrual symptoms (headache, diarrhea).
On the first day, the discharge was only brown, the second and third days were red, and by the fourth day, there was hardly any blood.
My partner and I had intercourse on September 20, but we used a condom.
I took a pregnancy test on the first day of my period (October 1) and it showed only one line.
It has been 15 days since we last had intercourse, and today (October 5) the pregnancy test still shows only one line.
Now, on the fifth day of my period, there is no blood left, just brown discharge, but my abdomen still feels bloated, as if blood is not being expelled.
Can you please advise if this rules out the possibility of pregnancy?

Joyce, 20~29 year old female. Ask Date: 2015/10/06

Dr. Lü Lizheng reply Obstetrics and Gynecology

The pregnancy test result is reliable 15 days after the sexual intercourse, indicating that there is no pregnancy.
Sincerely, Dr.

Understanding irregular menstrual cycles can be a complex issue, as they can be influenced by a variety of factors including hormonal imbalances, stress, lifestyle changes, and underlying health conditions. Based on your description, it seems that you are experiencing a change in your menstrual cycle, which has raised some concerns for you.

Firstly, it's important to note that menstrual cycles can vary significantly from month to month. A typical menstrual cycle ranges from 21 to 35 days, and variations can occur due to numerous factors. The symptoms you mentioned, such as a lighter flow, delayed onset, and changes in color (from brown to red), can be indicative of hormonal fluctuations. The presence of premenstrual symptoms like headaches and gastrointestinal issues can also be linked to hormonal changes.

Regarding your concerns about pregnancy, the fact that you used a condom during intercourse is a positive step in preventing unintended pregnancy. Condoms are generally effective when used correctly, but no contraceptive method is 100% foolproof. The negative pregnancy tests you conducted on both October 1st and October 5th, which showed only one line, suggest that pregnancy is unlikely. However, it’s important to consider that testing too early can sometimes yield false negatives, especially if your menstrual cycle is irregular.

The brown discharge you observed at the beginning of your period is often old blood that takes longer to exit the body, which is not uncommon. The fact that your bleeding stopped after a few days and was accompanied by a feeling of bloating could suggest that your body is experiencing some hormonal changes. This can lead to a sensation of fullness or pressure in the abdomen, which might feel like blood is not being expelled properly.

If you continue to experience irregular cycles, persistent bloating, or any other concerning symptoms, it would be advisable to consult with a healthcare provider. They may recommend further evaluation, which could include blood tests to check hormone levels, a pelvic exam, or imaging studies if necessary. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or other hormonal imbalances can contribute to irregular menstrual cycles and should be ruled out.

In summary, while your symptoms may not immediately indicate pregnancy, they do warrant further investigation to understand the underlying causes of your irregular menstrual cycle. Keeping a record of your menstrual cycle, including flow, symptoms, and any other relevant factors, can be helpful for your healthcare provider in diagnosing any potential issues. If you have any further questions or concerns, please do not hesitate to reach out for professional medical advice.
