Pregnant at 8 to 9 weeks; taking Le Xia tablets?
Hello Doctor: I have been taking laxatives for a long time before pregnancy to have regular bowel movements; however, after learning about my pregnancy, my constipation has worsened, and I am experiencing bloating and pain, which is very uncomfortable.
I have taken laxatives and have been able to have regular bowel movements.
I would like to ask if taking laxatives at 8 to 9 weeks of pregnancy could affect the fetus.
During my second pregnancy, a doctor at Chang Gung prescribed me "Bian Tong Le" (a Chinese herbal laxative).
Are these two medications safe for pregnant women? Thank you.

Dr. Yang Wangjun reply Obstetrics and Gynecology
Hello: The active ingredient in Laxative Tablets is Bisacodyl, while the active ingredient in Bowel Relief is Senna.
Both can help promote the secretion of water and electrolytes into the colon and stimulate the intestinal mucosa, leading to increased colonic motility and aiding in bowel movements.
Both are classified as Category C medications during pregnancy, so it is generally recommended to minimize their use.
Thank you for your message!

When it comes to taking laxatives during early pregnancy, it is essential to proceed with caution and consult with a healthcare provider. The use of laxatives can be a common concern for pregnant women, especially those who have a history of constipation. In your case, you mentioned that you have been taking laxatives like "Laxative Tablets" (樂下錠) for regular bowel movements and are now experiencing increased constipation and discomfort during your pregnancy.
Firstly, it is important to understand that constipation is a common issue during pregnancy due to hormonal changes, particularly the increase in progesterone, which can slow down the digestive system. Additionally, the growing uterus can exert pressure on the intestines, further contributing to constipation. While it is crucial to address constipation for your comfort and overall health, the safety of laxatives during pregnancy varies depending on the type of laxative used.
Laxatives are generally classified into several categories:
1. Bulk-forming laxatives (e.g., psyllium, methylcellulose) are considered safe during pregnancy as they work by absorbing water and increasing the bulk of the stool, which helps stimulate bowel movements.
2. Stimulant laxatives (e.g., senna, bisacodyl) can be effective but should be used with caution. While some studies suggest they may not pose significant risks, they can cause cramping and may lead to dehydration if not used properly.
3. Osmotic laxatives (e.g., polyethylene glycol, lactulose) are generally considered safe and work by drawing water into the bowel to soften the stool.
4. Saline laxatives (e.g., magnesium citrate) should be avoided unless specifically recommended by a healthcare provider, as they can lead to electrolyte imbalances.
In your case, since you are currently 8-9 weeks pregnant, it is crucial to consult your healthcare provider before continuing or starting any laxative. They can evaluate your specific situation and recommend the safest options for you. If "Laxative Tablets" and "便通樂" (Bian Tong Le) are stimulant laxatives, it may be advisable to switch to a bulk-forming or osmotic laxative instead.
Moreover, lifestyle changes can also help alleviate constipation during pregnancy. Increasing your intake of dietary fiber through fruits, vegetables, and whole grains, along with staying well-hydrated, can significantly improve bowel regularity. Regular physical activity, as permitted by your healthcare provider, can also promote digestive health.
In summary, while some laxatives may be safe during early pregnancy, it is essential to consult with your healthcare provider to determine the best course of action for your specific situation. They can provide guidance on safe laxative use and suggest alternative methods to manage constipation effectively. Your health and the health of your baby are of utmost importance, so open communication with your healthcare team is key.
