A hard lump the size of a BB pellet at the back of the neck?

In the past couple of days, I suddenly noticed a hard lump the size of a BB pellet under the hairline on the left side of my neck.
I asked a colleague to take a look, and the appearance seems normal.
It is painful but does not move.
Should I make an appointment with an ENT specialist at a major hospital, or should I observe it first before taking further action? Thank you.

Dr. Zheng Jueyi reply Otolaryngology

Hello Mei: The most common issue is lymphadenitis.
If there is pain, it may be due to acute inflammation.
It is advisable to visit a clinic for a medical examination by a physician and to receive appropriate medication treatment.
Wishing you safety and health.
Sincerely, Dr.

More Info

When you discover a hard lump behind your neck, particularly in the area just below the hairline, it can be concerning. The characteristics you've described—a firm, non-mobile lump that is painful—warrant careful consideration. Here’s a breakdown of what you should know and when it might be appropriate to see an ENT (Ear, Nose, and Throat) specialist.

Possible Causes of a Hard Lump
1. Lymphadenopathy: One of the most common causes of a hard lump in the neck is swollen lymph nodes. This can occur due to infections (viral or bacterial), inflammatory conditions, or even malignancies. If the lump is tender and associated with other symptoms like fever or sore throat, it may indicate an infection.

2. Cysts: Sebaceous cysts or epidermoid cysts can form in the skin and may feel firm. These are usually benign and can sometimes become inflamed or infected, leading to pain.

3. Lipomas: These are benign tumors made of fatty tissue. They are usually soft and movable, but in some cases, they can feel firmer.

4. Muscle Strain or Injury: Sometimes, muscle tension or strain in the neck can lead to localized swelling or a hard lump, particularly if there has been recent physical activity or trauma.

5. Other Conditions: Less commonly, conditions such as thyroid nodules or tumors may present as lumps in the neck area.

When to See an ENT Specialist
Given that the lump is hard, painful, and non-mobile, it is advisable to take the following steps:
1. Initial Observation: If the lump has just appeared and you have not experienced any other concerning symptoms (like significant fever, weight loss, or persistent pain), you might consider observing it for a few days. Monitor for any changes in size, shape, or associated symptoms.

2. Seek Medical Attention: If the lump persists beyond a week, increases in size, or if you develop additional symptoms (such as fever, night sweats, or unexplained weight loss), you should seek medical attention promptly. An ENT specialist can perform a thorough examination and may recommend imaging studies (like an ultrasound or CT scan) to better understand the nature of the lump.

3. Immediate Consultation: If you experience severe pain, difficulty swallowing, or any neurological symptoms (like numbness or weakness), do not hesitate to seek immediate medical care.

Conclusion
In summary, while some lumps can be benign and self-limiting, the characteristics you've described suggest that it would be prudent to consult with an ENT specialist, especially if the lump does not resolve quickly or if you have any additional symptoms. Early evaluation can help rule out serious conditions and provide peace of mind. Remember, it’s always better to err on the side of caution when it comes to new growths or changes in your body.
