Are you pregnant?

Doctor, last month my period lasted for more than two weeks.
I had an ultrasound, and there was nothing found.
After taking the medication prescribed by the doctor, my period came a few days later.
Now, two weeks after my period, I sometimes experience cramping in my lower abdomen, and it seems to be getting larger.
Is there a possibility that I am pregnant?

Niming, 10~19 year old female. Ask Date: 2015/10/08

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, if there has been no sexual intercourse during this period, the chance of pregnancy is very low.
However, if there has been sexual activity, it is advisable to check for pregnancy if your period is more than a week late.
Thank you! Wishing you peace and health, Dr.
Zhang.

When considering the possibility of pregnancy, it's essential to understand the symptoms and the implications of ultrasound results. Based on your description, there are several factors to consider.

Firstly, if your menstrual cycle is irregular, it can complicate the timing of ovulation and conception. Typically, ovulation occurs about 14 days before your next expected period. If you experienced a late period and then had symptoms such as abdominal pain and bloating, these could be indicative of various conditions, including pregnancy. However, they could also be related to other factors such as hormonal changes, stress, or even gastrointestinal issues.

The absence of visible gestational structures on an ultrasound shortly after a missed period is not uncommon. Early in pregnancy, especially within the first few weeks, the embryo may be too small to be detected via ultrasound. This is particularly true if you are still very early in your pregnancy, as the embryo typically becomes visible around 5-6 weeks of gestation. If you had an ultrasound and no gestational sac was seen, it might be too early to confirm pregnancy definitively.

The abdominal pain you are experiencing could be due to several reasons. If it is intermittent and not accompanied by heavy bleeding, it may not be a cause for immediate concern. However, if the pain is severe or accompanied by other symptoms such as heavy bleeding, it is crucial to seek medical attention. In some cases, abdominal pain can indicate an ectopic pregnancy, where the embryo implants outside the uterus, which can be a serious condition.

Regarding your question about the possibility of being pregnant, if you have missed your period and are experiencing symptoms such as abdominal pain, it would be wise to take a home pregnancy test. These tests are generally reliable and can provide a quick indication of pregnancy. If the test is positive, follow up with your healthcare provider for further evaluation and possibly another ultrasound to confirm the pregnancy and check for the location of the embryo.

If you are still unsure and your symptoms persist, it is advisable to consult with a healthcare professional. They can perform a blood test to measure the levels of human chorionic gonadotropin (hCG), the hormone produced during pregnancy, which can provide more definitive answers than a urine test. Additionally, they can assess your symptoms and provide guidance on the next steps, including further imaging if necessary.

In summary, while your symptoms could suggest pregnancy, it is essential to confirm this with a pregnancy test and follow up with your healthcare provider for a thorough evaluation. Early pregnancy can be accompanied by various symptoms, and understanding these in the context of your overall health is crucial for ensuring both your well-being and that of a potential pregnancy.
