Discomfort During Bowel Movements: Causes and Concerns - Gastroenterology and Hepatology

Recently, I've been feeling a soreness when having a bowel movement?

Hello, a couple of days ago I went on a trip and spent a long time sitting on a tour bus.
I didn't have a bowel movement for two days.
The day after I got home, I started to feel the urge to have a bowel movement.
During the process, I had to exert some effort, but it wasn't difficult to pass.
However, while straining, I experienced some discomfort, and I'm not sure if it's muscle soreness or something deeper.
For the past two days, I've had a persistent aching sensation, and I'm unclear about the source of this discomfort, which is around the anal area or possibly from within the anus.
This sensation is somewhat similar to the soreness experienced during menstruation when straining to have a bowel movement.
I would like to know if this could possibly be a sign of colorectal cancer or if it might be hemorrhoids.
However, I haven't noticed any blood or changes in my stool.

Dr. Lin Minghui reply Gastroenterology and Hepatology

Hello, experiencing irregular bowel movements with a sour sensation may be related to recent dietary habits and possible intestinal inflammation.
It is advisable to maintain moderate exercise, limit spicy foods, and increase fiber intake along with adequate hydration.
If symptoms persist, follow-up examinations at a clinic are recommended.
If there is blood in the stool, it is important to investigate the cause, which may include hemorrhoids, anal fissures, colitis, diverticulitis, polyps, or tumors.
Sincerely, Dr.

Understanding discomfort during bowel movements can be concerning, especially when accompanied by unusual sensations or changes in bowel habits. Based on your description, it seems you experienced discomfort after a period of travel and prolonged sitting, which may have contributed to your symptoms. Let’s explore the potential causes and concerns related to your situation.

Firstly, the discomfort you are feeling during bowel movements could be attributed to several factors. One common cause is muscle strain. When you are constipated or have not had a bowel movement for a couple of days, the muscles in the pelvic floor and around the anus may become tense or strained when you attempt to pass stool. This can lead to a sensation of soreness or discomfort, which you described as feeling "acid" or sore in the area. This is particularly common if you have had to exert more effort than usual to have a bowel movement.

Another possibility is hemorrhoids, which are swollen veins in the lower rectum and anus that can cause discomfort, especially during bowel movements. While you mentioned that you have not noticed any blood or changes in your stool, it is still possible to have hemorrhoids without visible bleeding. They can cause a feeling of pressure or soreness in the anal area, particularly after straining.

Additionally, the sensation you described could be related to anal fissures, which are small tears in the lining of the anus that can occur due to passing hard stools. These can cause sharp pain and discomfort during and after bowel movements, and while they often come with bleeding, not all fissures do.

Regarding your concern about colon cancer, it is understandable to worry about serious conditions, especially when experiencing discomfort. However, the absence of other symptoms such as blood in the stool, significant changes in bowel habits, unexplained weight loss, or persistent abdominal pain makes it less likely that your symptoms are related to a serious condition like colon cancer. Nonetheless, it is important to monitor your symptoms and consult a healthcare professional if they persist or worsen.

To alleviate your discomfort, consider the following recommendations:
1. Hydration: Ensure you are drinking plenty of water, as this can help soften your stool and make bowel movements easier.

2. Dietary Fiber: Incorporate more fiber into your diet through fruits, vegetables, and whole grains. This can help promote regular bowel movements and reduce straining.

3. Warm Baths: Taking warm baths can help relax the muscles around the anus and provide relief from discomfort.

4. Gentle Exercise: Engaging in light physical activity can stimulate bowel function and help alleviate constipation.

5. Avoid Straining: When you feel the urge to have a bowel movement, try to relax and avoid straining, as this can exacerbate discomfort.

If your symptoms persist for more than a few days, or if you experience any additional concerning symptoms, it is advisable to seek medical attention. A healthcare provider can perform a thorough evaluation, which may include a physical examination and possibly further diagnostic tests, to determine the underlying cause of your discomfort and recommend appropriate treatment.

In summary, while your symptoms may be uncomfortable, they are not necessarily indicative of a serious condition like colon cancer, especially in the absence of other alarming signs. However, staying vigilant and consulting a healthcare professional if symptoms persist is always a prudent approach to managing your health.
