Delayed menstruation?

Hello doctor, my girlfriend's last menstrual period was on August 28, and she was expected to get her next period on September 28.
However, today is October 11, and she still hasn't gotten her period.
We had sexual intercourse around October 1 or 2, and we used a condom throughout.
What could be the situation? Thank you.

wei, 20~29 year old female. Ask Date: 2015/10/11

Dr. Lü Lizheng reply Obstetrics and Gynecology

If condoms were used consistently, the likelihood of pregnancy is low.
However, to confirm, it is advisable to take a urine pregnancy test for peace of mind.
If the test is negative, it may just be irregular menstruation, and it is recommended to seek medical evaluation and treatment.
Sincerely, Dr.

Understanding delayed menstruation can be a source of concern for many individuals, especially when there are recent sexual activities involved. In your case, your girlfriend's last menstrual period was on August 28, and she was expected to have her next period on September 28. However, as of October 11, she has not yet menstruated. You mentioned that there was sexual intercourse around October 1 or 2, and that condoms were used throughout the encounter.
There are several potential reasons for a delayed menstrual cycle, and it is important to consider both physiological and psychological factors that may contribute to this situation.

1. Pregnancy: Although you used condoms, no contraceptive method is 100% effective. If the condom failed or was used incorrectly, there is a possibility of pregnancy. It is advisable for your girlfriend to take a home pregnancy test (HPT) to rule out this possibility. Most HPTs are reliable when taken after a missed period, but they can sometimes detect pregnancy earlier.

2. Stress: Emotional or physical stress can significantly impact the menstrual cycle. Factors such as anxiety, changes in routine, or significant life events can lead to hormonal imbalances that may delay menstruation.

3. Hormonal Imbalances: Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or other hormonal issues can cause irregularities in the menstrual cycle. If your girlfriend has a history of irregular periods, this could be a contributing factor.

4. Weight Changes: Significant weight loss or gain can also affect menstrual cycles. If your girlfriend has experienced any changes in her weight recently, this could be a reason for the delay.

5. Illness or Medications: Certain illnesses, especially those that affect the endocrine system, can lead to delayed menstruation. Additionally, some medications, including hormonal contraceptives, can alter menstrual patterns.

6. Age and Menstrual Cycle Variability: If your girlfriend is in her late teens or early twenties, it is normal for menstrual cycles to be somewhat irregular as the body adjusts to hormonal changes.
Given that it has been several weeks since her expected period, it would be prudent for her to take a pregnancy test as the first step. If the test is negative and her period does not arrive soon, or if she experiences other symptoms such as severe pain, unusual discharge, or significant changes in her health, it would be advisable for her to consult a healthcare provider. A medical professional can perform a thorough evaluation, including a physical examination and possibly blood tests, to determine the underlying cause of the delayed menstruation.

In summary, while there are various reasons for a delayed period, the most immediate action is to take a pregnancy test. If the result is negative and the delay continues, seeking medical advice would be the next best step to ensure her health and well-being.
