Polycystic Ovary Syndrome (PCOS)
Hello Doctor: My menstrual cycle is regular every month, but for over a year, my flow has been very light, lasting only about two days.
I have consulted a traditional Chinese medicine practitioner to help with my condition and have been tracking my basal body temperature, which shows both low and high temperatures indicating ovulation.
Since I have been married for almost a year and I am currently 28 years old, I would like to conceive.
I have arranged for medical examinations, including a hysteroscopy and a check on my fallopian tubes, both of which showed no issues.
However, blood tests revealed that my AMH level is 27, which is quite high, indicating that I have many immature follicles.
The doctor advised that if I want to get pregnant, I should do so before turning 30, as the quality of eggs tends to decline with age, reducing the chances of conception.
I have seen a doctor who suggested taking ovulation medication, but I ovulate every month; however, it may be that the follicles are immature.
I am also concerned that taking ovulation medication might thin my endometrial lining, making implantation more difficult in the future.
I would like to ask the doctor what I should do at this point.
Should I take the ovulation medication or not? Are there any methods to help mature the follicles and increase their size?

Dear Inquirer / 28-year-old Female User: Hello,
1.
You have polycystic ovary syndrome (PCOS) and have been married for over a year.
Currently, at 28 years old, you have plans to conceive.
I recommend that you work with an obstetrician-gynecologist to take ovulation medications to regulate your menstrual cycle and promote ovulation, which can increase your chances of conception.
2.
You may also consider integrating traditional Chinese medicine (TCM) for diagnosis and treatment to achieve a more effective outcome.
Whether to use solely Western medicine, solely TCM, or a combination of both depends on individual preferences and perspectives, as each approach has its unique characteristics.
3.
However, for those with polycystic ovary syndrome who are planning to conceive, trying a combination of Western and traditional Chinese medicine may enhance the success rate.
It is advisable to consult a qualified TCM practitioner for appropriate diagnosis and treatment, as they can provide more tailored care.
Finally, I wish you good health and success in your pregnancy.

Managing Polycystic Ovary Syndrome (PCOS) can be a complex journey, especially when it comes to enhancing egg quality and fertility. Given your situation, where you have regular menstrual cycles but experience low flow and have a high Anti-Müllerian Hormone (AMH) level, it’s essential to understand the implications of these factors and explore strategies to improve your chances of conception.
Understanding PCOS and Egg Quality
PCOS is characterized by hormonal imbalances that can lead to irregular ovulation, among other symptoms. A high AMH level, like your reported value of 27, indicates a higher number of immature follicles in the ovaries, which is common in PCOS. While this suggests a good ovarian reserve, it does not necessarily correlate with the quality of the eggs produced. Egg quality is crucial for successful fertilization and implantation, and it tends to decline with age.
Strategies for Enhancing Egg Quality
1. Lifestyle Modifications:
- Diet: A balanced diet rich in whole foods, lean proteins, healthy fats, and plenty of fruits and vegetables can help manage insulin levels and improve overall health. Consider a low-glycemic index diet to help regulate blood sugar levels.
- Exercise: Regular physical activity can improve insulin sensitivity, which is particularly beneficial for women with PCOS. Aim for a mix of aerobic and strength-training exercises.
- Weight Management: If you are overweight, even a modest weight loss of 5-10% can improve ovulatory function and fertility.
2. Medical Interventions:
- Ovulation Induction: If you are ovulating but producing immature eggs, your doctor may recommend medications like Clomiphene Citrate or Letrozole to stimulate the ovaries to produce higher-quality eggs. These medications can help ensure that the eggs that are released are more mature.
- Monitoring: Regular monitoring through ultrasounds can help track follicle development and ensure that you are producing mature eggs. This can guide the timing of intercourse or assisted reproductive technologies if needed.
3. Hormonal Balance:
- Metformin: If you have insulin resistance, Metformin may be prescribed to help improve insulin sensitivity and potentially enhance ovulatory function.
- Supplements: Some studies suggest that supplements like inositol can improve ovarian function and egg quality in women with PCOS. Omega-3 fatty acids and antioxidants may also support reproductive health.
4. Consultation with Specialists:
- It’s crucial to work closely with a reproductive endocrinologist or a fertility specialist who understands PCOS. They can provide personalized treatment plans based on your specific situation, including the potential use of ovulation induction medications and monitoring protocols.
Concerns About Medications
You mentioned concerns about taking ovulation medications and the potential for thinning the uterine lining. While some medications can affect the endometrium, careful monitoring and adjustments can help mitigate these risks. Your healthcare provider can guide you on the best approach to ensure that your uterine lining remains healthy for implantation.
Conclusion
In summary, while managing PCOS and enhancing egg quality can be challenging, there are several strategies you can adopt. Focus on lifestyle changes, consider medical interventions under the guidance of a specialist, and maintain open communication with your healthcare team. Remember, each woman's journey with PCOS is unique, and what works for one person may not work for another. With the right support and strategies, many women with PCOS successfully conceive and have healthy pregnancies.
