What problem is occurring with my gastrointestinal system?

Doctor, I have a question.
Last night when I had a bowel movement, I had to exert a little effort to pass the stool, but afterward, my stomach started to hurt, and it felt very hard.
The pain persisted until today, although it is less intense today.
Yesterday, the pain was continuous and cramp-like, while today it comes in intermittent waves.
When I checked my stool, it appeared normal, formed into separate pieces, initially long but then becoming shorter, and the color was normal.
Can you tell me what might be happening with my intestines?

Huang Liren, 10~19 year old female. Ask Date: 2015/10/15

Dr. Chen Qifeng reply Gastroenterology and Hepatology

Hello, sometimes fecal impaction can cause intestinal colic.
If one is unable to have a smooth bowel movement, it can easily lead to cramping or even spasms.
Sincerely, Dr.

It sounds like you're experiencing a combination of digestive issues that could be related to several factors, including diet, hydration, and possibly underlying gastrointestinal conditions. Based on your description, it seems you are dealing with abdominal pain, changes in bowel habits, and discomfort during bowel movements. Let's break down some potential causes and considerations.

1. Constipation: The need to exert effort during bowel movements can indicate constipation. This condition can be caused by a low-fiber diet, inadequate fluid intake, or lack of physical activity. When stool becomes hard and dry, it can lead to straining, which may cause abdominal pain and discomfort. The fact that you mentioned your stool was initially long but later became shorter could suggest that your bowel movements are becoming less regular or that you're experiencing incomplete evacuation.

2. Abdominal Pain: The abdominal pain you're experiencing could be due to several reasons. If the pain is crampy and intermittent, it might be related to bowel spasms, which can occur with constipation. On the other hand, if the pain is persistent and severe, it could indicate a more serious issue, such as an obstruction or inflammation in the intestines.

3. Dietary Factors: Consider your recent dietary habits. A diet low in fiber can lead to constipation, while a sudden increase in fiber without adequate hydration can also cause discomfort. Foods rich in fiber, such as fruits, vegetables, and whole grains, are essential for maintaining regular bowel movements. Additionally, ensure you are drinking enough water throughout the day, as hydration plays a crucial role in preventing constipation.

4. Possible Gastrointestinal Conditions: While your stool appears normal, persistent abdominal pain and changes in bowel habits could warrant further investigation. Conditions such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), or even gastrointestinal infections could present with similar symptoms. If your symptoms persist or worsen, it would be advisable to consult a healthcare professional for a thorough evaluation.

5. When to Seek Medical Attention: If you experience severe abdominal pain, blood in your stool, significant changes in your bowel habits, or if your symptoms do not improve with dietary changes and increased hydration, you should seek medical attention. A healthcare provider may recommend diagnostic tests, such as imaging studies or endoscopy, to determine the underlying cause of your symptoms.

In summary, your symptoms could be indicative of constipation or other gastrointestinal issues. It is essential to assess your diet, hydration, and overall lifestyle. If your symptoms persist or worsen, do not hesitate to consult a healthcare professional for a comprehensive evaluation and appropriate management. Remember, maintaining a balanced diet rich in fiber and staying hydrated are key components in promoting digestive health.
