Small white spots on the inside of the cheeks (similar to acne)?

Today, while licking the inside of my left cheek, I noticed two very small white bumps that resemble the white comedones that sometimes appear on the skin.
They are not painful or itchy, and they are not particularly rough.
I have never smoked or consumed alcohol.
I would like to ask the doctor if the term "leukoplakia" refers to a small area that has turned white, or if these small dots are considered leukoplakia.
Does the area where leukoplakia occurs become rough and dry?

Zhang, 20~29 year old female. Ask Date: 2015/10/17

Dr. Zheng Jueyi reply Otolaryngology

Hello: It is likely that the small salivary glands in the oral cavity are slightly enlarged.
If there is no significant pain or ulceration, you can observe it for now.
Leukoplakia is often associated with long-term chronic irritation, such as smoking or betel nut chewing.
If you are still concerned, you may seek medical attention for confirmation.
Wishing you peace and health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

The presence of small white bumps inside the cheeks can be attributed to several benign conditions, and understanding these can help alleviate concerns. The small white bumps you described, which are painless and not particularly rough, could be a few different things.

1. Fordyce Spots: These are small, painless, pale bumps that are actually sebaceous glands without hair follicles. They are completely normal and common, often appearing on the lips and inside the cheeks. They do not require treatment unless they cause cosmetic concern.

2. Mucosal Cysts: These are small, fluid-filled sacs that can develop in the oral cavity, often as a result of minor trauma or irritation. They are usually harmless and may resolve on their own.

3. Oral Lichen Planus: This is a chronic inflammatory condition that can present as white patches or bumps in the mouth. It may be associated with discomfort, but in some cases, it can be asymptomatic. If the bumps change in appearance or become painful, it would be advisable to consult a healthcare professional.

4. Leukoplakia: This condition presents as white patches in the mouth that cannot be scraped off. While often benign, leukoplakia can sometimes indicate precancerous changes, especially in individuals who smoke or use tobacco. Regular monitoring and consultation with a healthcare provider are recommended if leukoplakia is suspected.

Regarding your question about leukoplakia and whether it refers to a small area of whitening or small dots, leukoplakia typically refers to larger patches rather than small dots. The areas affected by leukoplakia can sometimes feel rough or thickened, but this is not always the case.

In terms of treatment, if the bumps are indeed Fordyce spots or mucosal cysts, no treatment is necessary. However, if you notice changes in the size, color, or texture of the bumps, or if they become painful, it is important to seek evaluation from a healthcare provider, preferably an oral surgeon or a dentist. They can provide a definitive diagnosis and rule out any serious conditions.

In summary, while small white bumps inside the cheeks can be concerning, many of them are benign and do not require treatment. Maintaining good oral hygiene, avoiding irritants, and monitoring any changes in the bumps are good practices. If you have further concerns or if the bumps persist, a visit to a healthcare professional is advised for peace of mind and appropriate management.
