Issues After Abortion

At six weeks pregnant, I underwent surgery on October 20.
The doctor did not prescribe any contraction medications for me.
Currently, I am using birth control pills and have a habit of smoking.
I returned to work three days after the surgery and stood for four hours.
Since the third day post-surgery, I have been experiencing bleeding and pain in my lower abdomen.
The pain lasts a long time and is often accompanied by nausea.
What could be happening? Is it related to smoking?

Shi, 20~29 year old female. Ask Date: 2015/10/24

Dr. Lü Lizheng reply Obstetrics and Gynecology

After the abortion procedure, abdominal pain three days later can have many possible causes, including: uterine inflammation, uterine contraction pain, retained products of conception, uterine congestion, gastrointestinal issues, internal bleeding, and more.
It is advisable to seek medical evaluation.
Sincerely, Dr.

After an abortion, it is not uncommon for individuals to experience a range of symptoms, including abdominal pain, bleeding, and nausea. These symptoms can be influenced by various factors, including the surgical procedure itself, hormonal changes, and lifestyle choices such as smoking.
Firstly, it is essential to understand that after an abortion, the body undergoes significant physical and hormonal changes. The uterus needs time to heal, and during this period, it is normal to experience some bleeding and cramping. This bleeding can vary in intensity and duration, and while light spotting is typical, heavy bleeding or prolonged pain should be evaluated by a healthcare provider.

In your case, experiencing abdominal pain and bleeding three days post-abortion could be a part of the normal healing process. However, if the pain is severe, persistent, or accompanied by heavy bleeding (soaking through a pad in an hour), it could indicate complications such as retained products of conception or infection. Nausea can also be a side effect of hormonal changes or could be related to other factors, including stress or anxiety about the procedure and its aftermath.

Regarding the use of birth control pills post-abortion, it is generally safe to start hormonal contraceptives after the procedure. However, it is crucial to follow your healthcare provider's recommendations regarding when to begin taking them. Birth control pills can help regulate your menstrual cycle and reduce the risk of unintended pregnancy, but they do not address any immediate post-abortion complications.

Your smoking habit can also play a role in your recovery. Smoking is known to affect blood circulation and can potentially delay healing. It may also exacerbate nausea and other gastrointestinal symptoms. If you are a smoker, consider discussing cessation strategies with your healthcare provider, as quitting can significantly improve your overall health and recovery process.

It is essential to monitor your symptoms closely. If you notice any of the following, seek medical attention promptly:
1. Severe or worsening abdominal pain: While some discomfort is expected, severe pain that does not improve may indicate a complication.

2. Heavy bleeding: If you are soaking through pads in an hour or passing large clots, this requires immediate evaluation.

3. Fever or chills: These could be signs of infection and should be addressed by a healthcare professional.

4. Persistent nausea or vomiting: If these symptoms are severe or do not improve, it is important to consult with a doctor.

In summary, while some symptoms post-abortion can be normal, it is crucial to differentiate between typical recovery signs and potential complications. If you have concerns about your symptoms or the impact of smoking on your recovery, do not hesitate to reach out to your healthcare provider for personalized advice and support. Your health and well-being are paramount, and addressing any issues early can lead to a smoother recovery process.
